About the job
- Graduate of an accredited occupational therapy program accredited by ACOTE.
- Hold a current state license as an Occupational Therapist.
- Have at least one year of experience as a licensed Occupational Therapist.
- Have reliable transportation, a valid driver’s license, and valid automobile insurance coverage.
- Be willing to travel daily for home visits within the service area.
- Demonstrate clinical assessment and treatment planning abilities.
- Know medical terminology and home health regulations.
- Have problem-solving and creative thinking skills.
- Have verbal and written communication abilities.
- Be able to work independently with minimal supervision.
- Have organizational skills and attention to detail.
- Be comfortable working in varied home environments.
- Have interpersonal skills for professional interaction with patients, families, and teams.
- Assess patients’ functional abilities and design personalized treatment plans.
- Develop adaptive strategies and solutions that support patients’ independence and safety.
- Provide patient education, hands-on training, and encouragement as patients progress toward greater confidence and independence in daily activities.
- Work one-on-one with patients in their homes and provide care in patients’ homes, assisted living facilities, or nursing homes.
- Collaborate with patients, families, and an interdisciplinary care team.
- Home health experience.
- Experience with adaptive equipment and home modifications.
- Knowledge of Medicare home health documentation.
