About the job
- An Associate's Degree or diploma from an accredited Licensed Practical Nurse program is required.
- At least one year of acute care, long-term care, or home health experience is required.
- A Licensed Practical Nurse license from the state licensing board is required.
- Cardiopulmonary Resuscitation certification from the American Heart Association is required.
- A valid driver's license is required.
- Auto insurance is required.
- Implement the nursing plan of care at the direction of the registered nurse.
- Provide home-based patient care based on physician orders and the established nursing plan of care.
- Collaborate with the Home Health registered nurse and multidisciplinary treatment team to individualize each patient's treatment needs and achieve defined goals for improved patient outcomes.
- Conduct patient psychosocial assessments to explore the physical, psychological, and social aspects of the patient and their conditions, obtain history, and elicit an accurate description of psychosocial needs.
- Contribute to developing discharge plans, identify the patient's post-discharge needs, and target appropriate resources for follow-up care in collaboration with the patient, family, and care team.
- Document patient care information and ensure timely and accurate completion of patient care records in accordance with policies and procedures.
- Two years of home health experience is preferred.
