Requirements
  • A college or university degree in a related field is required.
  • Knowledge and skills developed through 5-7 years of work experience in a related job discipline are required.
Responsibilities
  • Develops and updates resource lists to refer families for therapies, adult mental health and counseling services, and tips for commonly encountered challenges, such as applying for Social Security Insurance and requesting an Individualized Education Plan.
  • Documents developmental care in records.
  • Attends all virtual and/or in-person appointments of the Early Intervention Medical Diagnostic Team.
  • Coordinates the multidisciplinary visits of the Early Intervention Medical Diagnostic Team.
  • Conducts the intake process on each patient prior to their appointment.
  • Compiles all reports at the end of each appointment within expected deadlines.
  • Facilitates communications with service coordinators at Child and Family Connections centers pertaining to Early Intervention and requests all necessary paperwork.
  • Handles billing for all patients for the Early Intervention program through Qclaims.
  • Interprets, monitors, and records client progress according to measurable goals described in treatment and care plans during individual care coordination clinic visits.
  • Completes a social work assessment when consulted by a Developmental and Behavioral Pediatrician, primarily for Comer patients.
  • Analyzes potential problems in home or clinic environments.
  • Refers patients, clients, or families to community resources that assist recovery from mental or physical illness and provide access to financial assistance, legal aid, housing, job placement, or education.
  • Assesses individual needs, performs adaptive tests and psychological screening, provides family counseling, and participates as a member of a developmental team.
  • Identifies community and University resources and makes appropriate referrals.
  • Uses broad knowledge and experience to develop and coordinate programs related to emotional, social, and financial care, including family-centered services, treatment and education of youth, employment and financial support, or health insurance and provider services.
  • Serves as a liaison between schools, families, and community agencies.
  • Maintains relationships with social work departments within the University and with community agencies at local and state levels.
  • Delivers emergency and crisis intervention services.
  • Performs other related work as needed.
Desired Qualifications
  • An advanced degree or higher in Social Work from an accredited institution.
  • Five years of work experience as a Social Worker.
  • Experience working in a social service program utilizing evidence-based practice.
  • Experience working with children and/or families.
  • Early Intervention program experience and certification.
  • A license in the State of Illinois as a Licensed Social Worker or Licensed Clinical Social Worker.
  • Bilingual proficiency in Spanish.
