  • An active registered nurse state license is required.
  • A valid driver's license is required.

Responsibilities
  • Collaboraborate with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated patient care.
  • Assess, plan, direct, and evaluate each patient's care plan to ensure it meets the patient's needs.
  • Manage emergency situations promptly, precisely, and professionally.
  • Complete medication administration and treatment needs for assigned patients.
  • Provide nursing care in accordance with applicable regulations and company policies.
  • Prepare and maintain accurate and complete clinical and progress notes on a timely basis.
  • Provide supervision and oversight to patient care.
  • Utilize the Electronic Visit Verification system.
  • Prepare physician orders as applicable.
  • Participate in continuing education programs, patient care conferences, and scheduled staff meetings.
  • Perform initial and ongoing nursing assessments.
  • Notify the physician and branch management of significant changes in patient status and obtain order changes as necessary.
  • Assume responsibility for ongoing interdisciplinary assessment of the patient.
  • Participate in the branch on-call rotation as assigned.
  • Follow organizational policies and procedures regarding infection control, hazardous-waste disposal, and use of protective devices, as evidenced by documentation.
  • Perform other assigned job-related tasks and duties.
Desired Qualifications
  • Nursing supervisory or leadership experience is preferred.
  • Six months of home care experience is preferred.
