Requirements
  • Must possess and maintain an active Qualified Health Professional (QHP) CASAC-Advanced Status.
  • Must have a Bachelor's Degree in Human Services.
  • Must have at least 6 months of experience in the substance use, mental health, or human services fields.
  • Must complete OASAS Clinical Foundations Trainings 1 and 2.
  • Must have a valid New York State driver's license.
Responsibilities
  • Manage service programming in conjunction with the Reintegration Manager while carrying a small caseload.
  • Assist the Reintegration Manager with developing other program staff members.
  • Oversee the maintenance of individual case records in compliance with agency policy and licensing authority regulations.
  • Develop and assist other staff members with service planning and discharge planning.
  • Facilitate weekly didactic and process groups as needed.
  • Document individual progress in accordance with agency policy and procedure.
  • Meet with residents regularly to monitor progress and revise individual service plans as needed.
  • Make referrals, coordinate services, and maintain regular ongoing consultations with other providers.
  • Coordinate educational and vocational skill development.
  • Organize and maintain case records.
  • Assist with the admission process and orientation of new residents as needed.
  • Provide breathalyzer and toxicology screening as needed.
  • Respond to crises and emergencies.
  • Complete data input into the agency database.
  • Respond professionally and responsibly to incidents involving contagious diseases, including HIV, Hepatitis B, and tuberculosis.
  • Assist with the program's medication policies and procedures.
  • Be available for on-call responsibilities and 24-hour coverage when needed.
  • Participate in weekly staff meetings and case conferences.
  • Serve on agency committees as requested.
  • Integrate Trauma-Informed Care and its five principles into duties and interactions with staff, residents, and external partners.
  • Maintain compliance with Federal Confidentiality requirements under 42 CFR Part 2, HIPAA, Medicaid, HUD, and other applicable rules and regulations.
  • Maintain regular attendance at the work location.
