Requirements
  • An active clinical license as an RN, LPN, PT, PTA, OT, COTA, ST, or Clinical Social Worker is required.
  • Three to five years of clinical experience in acute care, post-acute care, or senior living is required.
  • Strong clinical assessment, critical thinking, and decision-making skills are required.
  • The ability to build referral relationships and achieve occupancy goals is required.
  • Experience with EMR/CRM systems is required.
  • Excellent communication, organization, and customer service skills are required.
  • Proficiency with Microsoft Office is required.
  • A valid driver's license and reliable transportation are required.
  • The ability to work independently, manage multiple priorities, and maintain a flexible schedule is required.
Responsibilities
  • Achieve occupancy and census goals.
  • Review referrals and clinical documentation to determine admission eligibility.
  • Conduct clinical assessments and coordinate admissions with internal teams.
  • Ensure smooth transitions between hospitals, skilled nursing, and other levels of care.
  • Triage hospitalized residents and coordinate appropriate placement or return to care.
  • Build and maintain relationships with hospital case managers, discharge planners, physicians, social workers, and other referral sources.
  • Conduct hospital visits, bedside assessments, rounds, and community outreach activities.
  • Track referrals, admissions, and professional contacts within CRM/EHR systems.
  • Collaborate with admissions, clinical, and operations teams on referral and census management.
  • Support outreach events, marketing initiatives, and community engagement efforts.
  • Assist with training and guidance of Admissions Coordinators.
  • Serve as backup to the Admissions Director and provide coverage as needed.
  • Participate in on-call, weekend, and emergency response responsibilities.
Desired Qualifications
  • A bachelor's degree is preferred.
  • Admissions, liaison, business development, or census management experience is preferred.
  • PointClickCare experience is preferred.
