Requirements
  • A minimum of one year of experience as a registered nurse in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • A current license to practice nursing in the state of employment is required.
  • Current cardiopulmonary resuscitation certification is required.
  • Current automobile insurance and a valid driver's license are required.
  • Knowledge of terminally ill patients and their families, hospice principles, the roles of disciplines providing hospice services, community resources, general nursing practice, managed care principles, and regulatory guidelines including Medicare, Medicaid, and ACHC is required.
  • Excellent patient assessment, oral communication, written communication, clinical documentation, and program management skills are required.
  • The employee must maintain transportation for work and be able to operate nursing assessment tools, computers, facsimile equipment, copiers, and a cell phone or beeper.
  • The employee must be able to work in patients' homes or institutional settings and use an automobile, with potential exposure to infections, communicable diseases, odors, blood, excreta, and hazardous materials.
  • The employee must support patients and families dealing with life-and-death issues, communicate effectively with people from all socioeconomic backgrounds, and perform varied tasks with flexibility, tact, maturity, cooperation, and composure.
Responsibilities
  • Serve as the primary point of contact for patients and families throughout the hospice journey.
  • Deliver and document skilled, hands-on nursing care according to each patient's individualized plan of care in the patient's home, assisted living setting, or other residence.
  • Perform comprehensive initial, ongoing, in-person, and telephonic registered nurse assessments and reassessments, and provide education to patients, family members, and caregivers.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Collaborate and coordinate care with physicians, nurse practitioners, licensed practical nurses, certified nursing assistants, social workers, chaplains, and other interdisciplinary team members.
  • Provide emotional, physical, and spiritual support, education, comfort, bereavement guidance, and caregiver training.
  • Educate patients and caregivers about disease processes, self-care techniques, end-of-life care, medications, nutrition, dietary needs, hospice philosophy and services, symptom and pain management, and end-of-life processes and expectations.
  • Communicate changes in condition and signs or symptoms of decline to appropriate providers, including nurse practitioners, primary care physicians, and hospice physicians.
  • Support home health, palliative, and hospice referrals and transitions between levels of care.
  • Maintain timely and accurate clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the patient's care plan requires team participation.
  • Follow infection-control, regulatory, compliance, and policy standards.
  • Participate in quality-improvement, scheduling, on-call, patient-support, and interdisciplinary-group activities, including clinically competent and responsive on-call services.
