Requirements
  • A high school diploma or equivalent is required.
  • Access to reliable transportation is required to facilitate travel throughout the service area and perform essential functions.
  • At least one year of experience within the last five years in an activities program in a healthcare setting serving a frail or elderly population, or completion of the employer's training program, is required.
  • Cardiopulmonary resuscitation and Alzheimer's certifications are essential, or the candidate must be willing to obtain them.
Responsibilities
  • Conduct large and small therapeutic recreational groups and individual recreational activities to improve or maintain participants’ cognitive, physical, social, psychosocial, spiritual, and artistic needs.
  • Contribute activity ideas for monthly activity planning calendars.
  • Provide educational and culturally competent activities, including exercise, arts and crafts, music appreciation, and spirituality.
  • Ensure participant choice and engagement when planning and conducting activities.
  • Build and foster a warm social atmosphere and support system for participants at the adult day health center.
  • Maintain recreational supplies and equipment according to procedure and report damaged or unsafe equipment to the supervisor.
  • Suggest supplies and equipment that may benefit the activity program.
  • Participate as a member of the interdisciplinary team and attend assigned team meetings.
  • Participate in individual or family meetings and case conferences as assigned.
  • Inform the supervisor of changes in participants’ conditions or participant complaints.
  • Complete assigned work reliably within identified time frames.
  • Document PACE center attendance daily as assigned.
  • Document participation in activities and other required records daily as assigned.
  • Complete activity assessments as needed and within the timelines required by PACE regulations.
  • Perform duties in accordance with organizational and program policies and procedures.
