Requirements
  • A bachelor's degree with major coursework in criminal justice, sociology, social work, psychology, counseling, or a related field.
  • A minimum of two years of direct-service experience with individuals with mental health concerns.
  • Experience working with individuals with forensic involvement and/or co-occurring substance use disorders is preferred.
  • A valid driver's license must be possessed and maintained, with a driving record in compliance with St. Luke's University Health Network policy.
  • The functional capacity to perform heavy work, including occasionally lifting or carrying up to 50 pounds and sitting, standing, and/or walking for at least six hours in an eight-hour workday.
  • Frequent stooping, crouching, and handling, and occasional fine manipulation are required.
  • Good physical and mental health, with visual and auditory acuity sufficient to provide comprehensive care.
Responsibilities
  • Meet with ACT participants in their homes and other community venues to provide in vivo services that improve quality of life and maintain community tenure, including assistance with activities of daily living, budget planning and money management, stable housing, benefits and entitlements, meal planning and grocery shopping, transportation services, side-by-side coaching and training, and transportation or accompaniment to community activities and appointments.
  • Provide medication services, including medication delivery and observation of ingestion, as directed by the team psychiatrist, nursing staff, and Team Leader.
  • Provide service coordination for an assigned group of clients, including developing, writing, implementing, evaluating, and revising treatment goals.
  • Provide individual and supportive counseling to assist with symptom management.
  • Provide rotating on-call crisis services, including primary and backup on-call responsibilities, as directed by the Team Leader.
  • Complete timely documentation of all services using the electronic health record in accordance with program and Network standards.
  • Work weekends, evenings, and holidays on a rotating schedule as directed by the Team Leader.
  • Cross-train with other ACT specialties to provide services such as housing support and vocational support.
  • Develop relationships with community employers and assist participants with matching to jobs in their chosen work areas.
  • Provide on-the-job support and coaching in line with the Supported Employment model.
  • Assess participants' educational and work history and their goals for future vocational engagement.
  • Develop plans to help ACT participants reach their vocational goals.
  • Provide educational and support groups addressing participants' vocational and educational goals.
  • Provide primary support to ACT participants and cross-train other ACT team members to help participants access housing-related supports and resources.
  • Perform other assigned duties.
Desired Qualifications
  • A degree in rehabilitation counseling.
  • Experience working with individuals with forensic involvement and/or co-occurring substance use disorders.
