Requirements
  • Graduate of an approved school of professional nursing.
  • Have and maintain a valid clinical license according to state requirements or applicable compact nurse license rules.
  • Have previous experience practicing under the professional license for this position; this may be deferred with review and approval from the vice president of operations or regional president.
  • Have at least two years of nursing experience within the immediate previous 36 months.
  • Have at least one year of home health experience.
  • Demonstrate knowledge and understanding of federal, state, and local laws and regulatory guidelines governing the operation of a home care office.
  • Have and maintain CPR certification, as required by the position.
  • Have and maintain a valid state driver's license in the current state of residence.
  • Have and maintain automobile liability insurance as required by law.
  • Have basic demonstrated technology skills, including operating a mobile device.
  • Maintain dependable transportation in good working condition and be able to drive safely in all types of weather conditions.
  • Demonstrate oral and written communication, fluency in English, and active listening.
  • Demonstrate intermediate technology skills, especially Microsoft Office, including Word and Excel.
  • Attend the office, except in designated full-time remote positions; occasional remote work for temporary personal needs or crises does not waive this essential function.
Responsibilities
  • Ensure that home health services provided to patients are coordinated according to acceptable standards of practice and company procedures.
  • Review and approve patient information submitted by the licensed professional during start-of-care, recertification, resumption-of-care, or evaluation visits.
  • Facilitate relationships among physicians, referral sources, patients, caregivers, and employees.
  • Schedule patients with field clinicians and communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care.
Desired Qualifications
  • Have previous experience in a management position.
  • Have previous experience using Homecare Homebase (HCHB).
  • Be bilingual in English and Spanish to facilitate clear communication with patients and caregivers, improve service delivery, and support positive patient outcomes.
