Requirements
  • Current Registered Nurse registration with the College of Nurses of Ontario and entitlement to practice without restrictions.
  • A minimum of 5 years of experience as a practicing Registered Nurse.
  • Comprehensive patient health assessment skills.
  • Understanding of the principles of community nursing practice.
  • Proven experience leading and managing a team.
  • Knowledge of clinical documentation standards and practice guidelines.
  • A valid driver's license and access to reliable transportation.
  • A current, clear Vulnerable Sector Screening.
  • Commitment to professional development in clinical practice relevant to home health care.
Responsibilities
  • Supervise and support Registered Nurses and Registered Practical Nurses to ensure practices comply with legislated standards of practice and company policies and procedures.
  • Respond to and advise on or resolve concerns from community staff, patients and families, and other team members.
  • Provide ongoing leadership through regular huddles, face-to-face meetings, and one-on-ones.
  • Initiate patient safety incident investigations and document them to ensure timelines and action plans are met.
  • Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
  • Engage patients in problem-solving using a collaborative approach, provide health teaching, build rapport and trust, and identify patient health care needs, strengths, capacities, and goals.
  • Engage staff in problem-solving by offering education, coaching, and mentoring to resolve problems.
  • Maintain the knowledge, skills, and attitudes required to provide safe, efficient, and effective service across all areas of practice by keeping abreast of changes in the practice setting that affect scope of practice.
Desired Qualifications
  • Canadian Nurses Association certification as a Community Health Nurse (CCHN(C)) is considered an asset.
  • Excellent organizational, interpersonal, and teaching skills.
  • Effective critical-thinking and problem-solving skills.
