Requirements
  • A current pharmacist license issued by the Oregon State Board of Pharmacy.
  • Training or experience in a specialized area may be required at the time of recruitment.
  • The pharmacist must provide safe and proper distribution and use of medications for inpatient and outpatient patients across the age spectrum.
  • The pharmacist must demonstrate strong communication skills for patient education and collaborate with inpatient and outpatient interdisciplinary teams.
Responsibilities
  • Identify, resolve, and document complex therapeutic problems using advanced knowledge for a specific patient population.
  • Respond accurately to drug-information requests from physicians, nurses, patients, and patients’ families; independently review and interpret the literature and apply the information to specific patient-care situations.
  • Monitor therapeutic response and medication adverse effects to guide drug therapy, consult providers about identified questions or problems, and report adverse drug reactions, drug defects, and medication errors.
  • Direct the work of students and residents and monitor and evaluate their performance.
  • Influence therapy standards through peer-level relationships with attending medical staff and serve as a drug authority in a specialized practice area.
  • Assist in developing treatment guidelines for patients with specific diagnoses and document activities in the patient medical record, pharmacy monitoring profile, drug-use evaluation data collection, or adverse-drug-reaction reporting program.
  • Interpret and evaluate physicians’ orders and prescriptions for accuracy, completeness, and appropriateness.
  • Verify the accuracy and appropriateness of medications prepared and dispensed from the central pharmacy.
  • Supervise pharmacy technicians’ activities and assume responsibility for duties performed in assigned work areas.
  • Participate in intradepartmental and interdepartmental education programs and contribute to peer development.
  • Assist with medication reconciliation, educate patients about home medication regimens, coordinate discharge medication plans with the interdisciplinary team, and document activities in the patient medical record.
  • Provide medication-therapy recommendations during prescribing decisions and, when appropriate, make independent medication-therapy decisions in cooperation with healthcare team members.
  • Facilitate medication reconciliation, discharge counseling, and medication delivery through coordination of care from admission to discharge.
Desired Qualifications
  • A Bachelor of Science in Pharmacy or Doctor of Pharmacy degree plus a practice residency or specialist certification such as Board Certified Pharmacotherapy Specialist, Board Certified Critical Care Pharmacist, or Board Certified Oncology Pharmacist.
  • A postgraduate year-two specialty residency in critical care or emergency medicine.
  • Vaccination certification.
  • Emergency department experience, critical care experience, and familiarity with Code Blue response.
  • Experience with outpatient pharmacy clerical rotation, transitions of care, discharge medication education, surgical or internal medicine services, pain management, and medication reconciliation.
