Requirements
  • Must be at least 18 years of age.
  • A high school diploma or educational equivalent is required.
  • Must hold or obtain and maintain a valid driver's license with no more than five points.
  • Ability to meet the essential physical demands of the position, including frequent walking, sitting, standing, bending, twisting, stooping, kneeling, crouching, pushing, pulling and reaching with hands and arms; use hands to handle, finger or feel objects, tools or controls; assist with lifting or moving individuals of any weight with assistance of mechanical lifts or other equipment and/or push individuals in wheelchairs; lift and/or move up to 25 pounds on occasion; and work with and implement emergency interventions as required by a person's Behavior Support Plan, including lifting or moving individuals of any weight with or without assistance of equipment and/or other staff.
  • Effective written and oral communication skills, including the ability to appropriately document services provided for billing and tracking purposes.
  • Enthusiasm for working with people with intellectual/developmental disabilities, willingness to take initiative, and ability to demonstrate positivity, patience, empathy and compassion in all interactions.
  • Ability and willingness to assist with a variety of supports and activities of daily living, including regular exposure to and handling of various bodily fluids.
  • Basic computer skills, including email and use of Workday and related systems such as Digital Agency to complete tasks.
  • Must hold or obtain and maintain certifications and trainings required by program assignment and persons supported, including Non-Crisis Invention training, American Heart Association CPR/First Aid certification, New Jersey Medication Administration certification and Positive Behavior Supports training within 120 days of appointment.
  • Ability to develop and maintain professional relationships in alignment with the organization's mission and vision.
  • Ability to remain calm in stressful situations and work in a high-energy, fast-paced environment.
  • Ability to work beyond regularly scheduled hours or shifts and/or report to work as scheduled on holidays, during inclement weather or during other emergencies to maintain minimum program coverage, unless otherwise authorized.
Responsibilities
  • Assist individuals in maximizing their strengths, interests and abilities, uphold their personal rights, provide emotional support and build positive relationships.
  • Support people in meeting personal goals that characterize quality of life, use person-first language, encourage socializing and facilitate activities that provide social capital and community connections.
  • Provide overall supervision, protection and care of individuals at all times, maintain program guidelines for their protection, facilitate fire-safety learning and training, and identify and appropriately escalate signs of potential harm to self or others.
  • Ensure health, wellness and safety through appropriate supervision and crisis management while adhering to applicable policies, procedures and regulations.
  • Provide support in a variety of settings according to individualized service and behavior support plans, using support skills for people with varying levels of need and demonstrating applicable developmental-disability standards.
  • Implement person-centered individualized plans, including Behavior Support Plans, Individualized Service Plans, Staff Action Plans and Life Plans, according to specified supervision levels and safeguards.
  • Maintain a clean, safe and organized therapeutic environment.
  • Use safe and appropriate handling procedures and implement infection-control protocols.
  • Provide personal care and assist with activities of daily living, including intimate care and personal hygiene such as bathing, dressing and toileting, as well as laundry and housekeeping.
  • Teach and develop habilitative and rehabilitative skills.
  • Support person-centered advocacy.
  • Assist with communication, self-expression and dispute resolution among individuals.
  • Facilitate ethical decision-making.
  • Supervise or plan and prepare balanced meals and snacks in accordance with individual dietary restrictions and guidelines, including modified diets and liquid consistencies.
  • Transport and escort individuals to recreational activities, medical appointments, home visits, volunteer or employment sites, and participate in establishing schedules for volunteer opportunities, community activities and therapeutic activities.
  • Assist people with making purchases and safeguard personal allowance funds and other provided funds.
  • Administer medication as prescribed and in accordance with applicable guidelines.
  • Assist with medication inventory, medication-cabinet checks and over-the-counter inventory, and reorder medication or supplies as needed.
  • Assist with scheduling medical appointments and ensure follow-up appointments are scheduled and attended in a timely manner.
  • Use petty cash and house funds, including store cards, and maintain purchase documentation and receipts according to applicable rules in coordination with the supervisor.
  • Operate an agency vehicle to transport individuals to and from programs, residences, community or other locations, and update related logs and maintenance sheets while coordinating appointments with the supervisor.
  • Develop specialized clinical knowledge for person-specific needs, including supporting adults, challenging behaviors, autism spectrum disorders and complex medical needs.
  • Implement Behavior Support Plans, behavior guidelines and protocols as specified in applicable plans.
  • Collect, record and summarize data on individuals' observable behavior.
  • Observe and report changes in behavior and other signs and symptoms to the clinical and leadership team.
  • Communicate by phone, email and in person with circles of support, including families and external providers, to provide updates or obtain information as directed by clinical or program leadership.
  • Serve as a member of the support team by providing feedback, communicating programmatic needs and maintaining a solution-oriented approach during conflicts.
  • Complete and maintain required trainings and certifications, including specialized training based on program and position, and apply the skills.
  • Review communication logs daily and enter events, progress notes, services and information; communicate pertinent information between team members and shifts using prescribed systems; and collect and enter data using an electronic health record and/or medication record such as Digital Agency.
  • Review and complete required paperwork and documentation for compliance and billing, including notifications, logs, incident reports, file updates, maintenance requests, phone calls and incident tracking.
  • Use foundational teaching skills, including Explain, Model, Observe and Evaluate, task analysis, essential skills, supports and modifications, and progressive teaching.
  • Understand behavior as communication and use person-centered thinking to support positive interactions and prevent escalation.
  • Report maintenance, upkeep and safety concerns at assigned facilities to the Program Supervisor or designee.
  • Consult with program leadership, clinical staff and direct support staff to ensure quality care and attend and actively participate in team meetings.
  • Use approved organizational and New Jersey developmental-disability training techniques, including verbal, environmental and physical interventions such as takedowns, to respond to challenging or self-injurious behavior and ensure the health and safety of supported individuals, self and others.
  • Provide support or coverage across assigned regions and service-delivery settings, including off-site locations such as hospitals, and remain on shift or report to another program to maintain minimum coverage levels.
  • May participate in developing treatment plans.
  • Comply with federal, state, local and other applicable regulatory requirements, including the Health Insurance Portability and Accountability Act.
  • Perform other assigned duties.
