Family Practice Center and Sylvan Health expand access to personalized nutrition care across Central Pennsylvania.

Family Practice Center and Sylvan Health expand access to personalized nutrition care across Central Pennsylvania. Sylvan Health Sep 10, 2026, 10:00 ET Collaboration brings evidence-based nutrition support to patients across 41 locations, with 95% patient satisfaction and 97% retention from initial visit to follow-up. SELINSGROVE, Pa., Sept. 10, 2026 /PRNewswire-PRWeb/ - Family Practice Center (FPC), a leading provider of family medicine across Central Pennsylvania, and Sylvan Health, a healthcare company expanding access to personalized nutrition care, announced their collaboration to make evidence-based nutrition support more accessible to patients across FPC's network. The program is now available across 41 Family Practice Center locations spanning 12 counties in Central Pennsylvania, supporting more than 200 providers and their family medicine patients. Through Sylvan Health, FPC patients can access personalized nutrition care from registered dietitians as an extension of their existing care team, helping address nutrition-related needs alongside their broader health goals. The collaboration is designed to make nutrition support a practical part of routine primary care, particularly for patients managing chronic care conditions, working toward healthier lifestyles, or looking to maintain their health and quality of life as they age. Early results demonstrate strong patient engagement with the program. 95% of patients report satisfaction with their nutrition care, while 97% return for a follow-up visit after their initial appointment, highlighting both the value patients place on personalized nutrition support and their willingness to continue care. "At FPC, we believe the best healthcare doesn't just treat illness, it helps prevent it. By expanding access to nutrition support, we can give our patients practical, sustainable tools to build healthier lifestyles, manage chronic conditions, and maintain their quality of life as they age," said Benjamin Williard, CEO of Family Practice Center. Sylvan Health's model integrates registered dietitians into existing clinical workflows, allowing providers to extend the care available to their patients without having to build and manage a nutrition program internally. The model supports referrals, scheduling, documentation, and billing while giving patients access to individualized nutrition guidance. "Primary care providers are on the front lines of helping patients prevent and manage chronic disease, but they don't always have the time or resources to provide the nutrition support that can make a meaningful difference," said Rhodes Amaker, CEO and Co-Founder of Sylvan Health. "Our work with Family Practice Center demonstrates what's possible when nutrition becomes a seamless extension of the care team, giving patients accessible, personalized support while making it easier for providers to deliver more comprehensive care." Family Practice Center serves patients through Central Pennsylvania, providing primary and family medicine to people across generations, from newborns to older adults. The collaboration with FPC reflects Sylvan Health's broader mission to make personalized, evidence-based nutrition care a standard part of healthcare. Sylvan's model is designed to integrate nutrition into existing care plans and workflows while delivering measurable value for patients and providers. About Family Practice Center Family Practice Center, PC is a physician-owned primary care organization serving Central Pennsylvania.
