About the Company

Bloom Healthcare

Physician-led house-call primary care for seniors

Bloom Healthcare provides in-home primary care for seniors, operating an employee-owned network that delivers house calls and medical services directly to homebound patients. The core team includes physicians and a staff of over 75 medical providers, who have completed more than 700,000 patient visits. In addition to primary care, Bloom runs Bloom Hospice to offer end-of-life care in the community. The company collaborates with home health agencies in the Front Range to ensure comprehensive, accessible care with quick care-plan approvals and continuous medical support in a patient’s own home. The distinct model centers on physician-led, home-based care for seniors who struggle to access traditional facilities, enabling them to stay independent and reducing the likelihood of hospitalization. The goal is to improve seniors’ health and independence by bringing high-quality, in-home medical and end-of-life care to their communities.
