Responsibilities
  • Conduct scheduled field and community-based visits throughout an assigned geographic territory.
  • Meet professionally with individuals and families in residential and community settings.
  • Gather, verify, and document information in accordance with established program protocols.
  • Observe and document circumstances related to the health, safety, and general welfare of assigned individuals.
  • Identify and appropriately report potential concerns in accordance with established procedures.
  • Maintain professional boundaries while demonstrating empathy, cultural awareness, and respect.
  • Conduct follow-up activities and maintain appropriate communication with program participants and designated stakeholders.
  • Prepare clear, objective, detailed, and timely electronic case notes and field reports.
  • Maintain complete, accurate, and confidential case documentation.
  • Utilize mobile devices, electronic case-management systems, and web-based applications to document field activities.
  • Manage an assigned workload while meeting required visit, documentation, and reporting deadlines.
  • Protect confidential and sensitive information at all times.
  • Follow all applicable federal, state, client, program, and company policies and procedures.
  • Participate in required training, meetings, and continuing program instruction.
  • Travel to other geographic areas for temporary assignments when operationally required.
  • Work with children, families, vulnerable populations, underserved communities, or individuals requiring social or community-based support services.
  • Travel daily to multiple field locations and throughout the assigned state or service territory.
  • Perform occasional out-of-state and overnight travel based on operational needs.
  • Independently manage daily field schedules, appointments, travel routes, and reporting deadlines.
  • Sit, stand, walk, and drive for extended periods; enter and exit vehicles frequently; navigate residential and community locations; occasionally lift or carry materials weighing up to 25 pounds; and regularly use a laptop, tablet, smartphone, and other mobile technology.
  • Work occasional evenings, weekends, extended days, and overnight assignments when required.
Desired Qualifications
  • An associate's or bachelor's degree in Social Work, Human Services, Psychology, Sociology, Behavioral Health, Criminal Justice, Family Studies, Healthcare, Education, Counseling, or a related field.
  • Previous experience in social services, case management, family services, community outreach, behavioral health, child or family support, healthcare, education, or a related human-services environment.
  • Experience conducting home visits, field visits, community assessments, wellness checks, or client interviews.
  • Experience working with children, families, vulnerable populations, or culturally diverse communities.
  • Experience maintaining case files, case notes, assessments, or other confidential documentation.
  • Experience working with a government agency, government contractor, nonprofit organization, or government-funded community program.
  • Bilingual or multilingual communication skills.
  • Previous federal background investigation, suitability determination, or security clearance experience.
