Requirements
  • Experience as a Music Practitioner or graduation from a program focused on music as an intervention for health and wellness.
  • Board certification as a Music Therapist by the Certification Board for Music Therapists.
  • Musical intelligence and mastery of at least one instrument.
  • Understanding of hospice philosophy and comfort providing specialized care to terminally ill patients.
  • Effective written and verbal communication skills.
  • Ability to follow safety rules and regulations, including Vulnerable Adult/Abuse Policy, HIPAA, Resident Rights, Universal Precaution, AWAIR, OSHA, and Right to Know.
  • Valid driver's license.
Responsibilities
  • Lead and assist patients in music therapy interventions.
  • Reduce symptoms such as anxiety, depression, isolation, and agitation through music.
  • Educate the clinical team on music therapy appropriateness and contraindications.
  • Maintain accurate clinical notes, charts, and coordination notes within 24 hours of sessions and update them as needed.

Moments Hospice provides Medicare-certified in-home hospice and palliative care across Minnesota, Wisconsin, Iowa, and select metros, delivering services wherever the patient calls home. It develops a joint, individualized care plan within seven days in collaboration with the patient, family, and treating facilities, and offers routine home care, continuous crisis care, general inpatient care, respite care, plus bereavement and spiritual support. The Moments Way centers on coordinating care with patients, families, and referral partners and uses a holistic approach to address physical, mental, and spiritual needs. Its goal is to improve quality of life for terminally ill patients, support families through grief, and raise the standard of hospice care, aided by the Moments Hospice Foundation that fulfills final wishes.
