Requirements
  • At least 5 years of post-residency experience.

Responsibilities
  • Provide leadership, oversight, and support for the team to execute daily priorities that enable high-quality care, a strong patient experience, team engagement, and a best-in-class culture.
  • Set the standard for care teams and Care Centers by executing the Hopscotch care model, key workflows, and processes while maintaining commitment to results, quality, and excellence.
  • Manage providers, including ensuring completion of administrative tasks, appropriate use of indirect care hours, panel management, and documentation.
  • Partner with the Care Center Manager to achieve target metrics and outcomes for patient experience, STARS/HEDIS, care coordination, utilization, and cost of care through clinical care team collaboration and engagement.
  • Provide high-quality patient care within Hopscotch's comprehensive and interdisciplinary care model for a dedicated patient panel.
  • Provide direct oversight of the clinical care team, including the Advanced Practitioner, Medical Assistant, Care Manager, Community Health Worker, and other team members.
  • Support the mission and vision through leadership in talent and culture, clinical innovation, compliance, and best practices.
  • Represent Hopscotch Health in the community and provide community-based education, engagement, and participation in health fairs and conferences.

Founded in 2021, Hopscotch serves more than 15,000 patients across the rural Southeast, concentrated in western North Carolina. Investors have hard numbers to point at: a Net Promoter Score of 89, patient retention above 90%, medical loss ratio improvement of more than 25 percentage points over two years and profitable operations in western North Carolina.
