Requirements
  • A Master's degree in Physical Therapy from an accredited program is required.
  • A current physical therapist license in the state of employment, or eligibility to obtain one, is required.
  • Current CPR certification is required.
  • Strong communication, assessment, and problem-solving skills are required.
  • The ability to work independently in a variety of home environments is required.
  • A valid driver's license, auto insurance, and reliable transportation are required.
Responsibilities
  • Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
  • Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
  • Adjust treatment plans and techniques as needed to achieve optimal outcomes.
  • Collaborate with the interdisciplinary home health team, including nurses, occupational therapists, speech therapists, and social workers, to provide coordinated care.
  • Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
  • Document all visits, assessments, and progress notes accurately and in a timely manner.
  • Support patients in maximizing their independence and improving quality of life in their homes.
Desired Qualifications
  • At least one year of experience in a hospital, home health, or outpatient rehabilitation setting is preferred but not required.
  • Experience delivering in-home therapy is preferred.
  • Experience with geriatric, orthopedic, or neurologic rehabilitation is preferred.
  • Experience with post-acute care or chronic condition management is preferred.
  • Experience working as part of a mobile or interdisciplinary care team is preferred.
