Responsibilities
Conduct detailed interviews to identify each client's psychological, medical, and social needs.
Develop specific, step-by-step care plans to help clients reach their goals.
Provide individual, group, and family psychoeducation-oriented care using various professional treatment modalities.
Coordinate individualized discharge planning for patients with physicians.
Communicate legal requirements for case management with physicians and nursing personnel.
Document treatment provided to patients in the medical record.
Report incidents of suspected abuse using prescribed forms and protocols from the relevant community or state agency.
Link patients to housing, food assistance, healthcare, and financial aid programs.
Document all client interactions and maintain accurate, confidential files.

We are looking for an empathetic, organized, and driven master's Level Case Manager. In this role, you will assess client needs, design individualized care plans, and connect individuals with critical community resources. You will advocate for clients and help them navigate complex support systems. This position will work with adult/gero acute patients.
