Requirements
  • A current Registered Nurse license in the state of employment is required.
  • At least one year of experience as a Registered Nurse in an acute care setting is required.
  • The ability to communicate effectively with patients, families, physicians, and co-workers is required.
  • The ability to remain calm, patient, and accommodating while working with patients is required.
  • Knowledge of nursing best practices is required.
  • The ability to make appropriate nursing judgments is required.
  • The ability to identify situations and handle them with the best possible solution is required.
  • A detail-oriented and observant work style is required.
  • A disciplined work ethic with the ability to prioritize tasks and work in a timely manner is required.
  • The ability to follow directions and work as a team member is required.
  • A valid driver’s license and auto insurance listing the employee as a driver are required.
  • The employee must meet all physical position requirements and provide evidence of a current negative tuberculin skin test or x-ray, or receive TB testing at orientation, followed by annual testing unless prior documentation is available.
  • The employee must be able to perform the listed physical job functions and may be exposed to bloodborne pathogens.
  • Reading, speaking, and writing English are required.
  • Computer or PDA usage, communication skills, hearing, and vision sufficient for close work are required.
Responsibilities
  • Oversee the overall care of each patient from the beginning to the end of the episode.
  • Update all physician orders in the patient chart accordingly.
  • Notify the physician of significant changes in the patient’s condition.
  • Manage multidisciplinary care when applicable.
  • Provide skilled nursing care in the patient’s home as ordered by the attending physician.
  • Observe and monitor patient conditions.
  • Perform OASIS assessments to develop individualized plans of care and adjust them as patient needs change.
  • Administer medications as prescribed by the physician.
  • Reduce rehospitalizations by front-loading visits for high-risk patients.
  • Promote continuity of care through appropriate admissions, transfers, and discharges.
  • Counsel patients and families on disease or injury processes and their management.
  • Incorporate patients and families into the development of the plan of care.
  • Teach patients preventive measures and decision-making practices to help reduce rehospitalizations.
  • Listen to patients and family members.
  • Oversee and supervise the total care provided by nurse aides and Licensed Practical Nurses.
  • Evaluate patient treatments and medical conditions according to the plan of care.
  • Determine whether the level of care provided meets patient needs.
  • Communicate supervisory-visit results to Licensed Practical Nurses and nurse aides.
  • Complete clinical documentation in accordance with agency protocols and Medicare and federal guidelines.
  • Document all treatment, assessment, and patient-education activities.
  • Maintain an active Registered Nurse license.
  • Communicate schedule changes outside the normal schedule to the scheduler.
  • Participate in on-call requirements and case conferences.
  • Attend in-service training and mandatory agency meetings.
  • Drive to patients’ primary locations for scheduled visits.
  • Attend assigned visit locations daily.
  • Document subsequent, discharge, evaluation or recertification, and resumption-of-care visits within 24 hours.
  • Document start-of-care visits within 48 hours.
  • Complete and submit required documentation within company timeframes.
  • Follow the plan of care within the Registered Nurse scope of practice.
  • Fulfill Registered Nurse visits in the assigned geographic area according to patient needs and federal and state requirements.
