Requirements
  • A bachelor's degree from an accredited college or university in social work, human services, or a related field.
  • At least 3 years of experience with highly vulnerable populations in a nonprofit, social services, or workforce development setting.
  • Demonstrated experience serving at least two of the following populations: justice-involved or re-entry individuals, survivors of domestic violence, people experiencing homelessness, or transitional age youth.
  • Experience participating in or facilitating multidisciplinary case conferences.
  • Strong knowledge of and experience applying trauma-informed care, healing-centered engagement, harm reduction, motivational interviewing, strengths-based case management, and crisis intervention.
  • Ability to work effectively in multidisciplinary team environments with clinical staff, youth counselors, gang intervention specialists, and instructional staff.
  • Skills in conflict resolution, de-escalation, and safety planning across diverse and sometimes high-risk participant circumstances.
  • Ability to maintain professional case records while balancing a high volume of participant contacts.
  • Proficiency in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook.
  • Proficiency in Google Suite, including Gmail, Drive, Docs, Sheets, Slides, and Meet.
  • Ability to use widely supported internet browsers.
  • Access to reliable transportation.
  • Ability to travel to meet with clients, between St. Joseph Center sites, and to community partner locations as needed.
  • Satisfactory completion of a background check is required for employment.
Responsibilities
  • Conduct comprehensive, trauma-informed intake assessments and develop individualized, goal-oriented service plans for participants across all three cohort tracks, identifying barriers and strengths.
  • Provide ongoing individualized case management through regular one-on-one meetings, check-ins, and responsive outreach while maintaining meaningful contact with participants across all three cohorts.
  • Provide advocacy and linkage to needed or supportive services and resources, including mental health services, rental assistance, food access, benefits enrollment, re-entry legal aid, childcare, immigration legal services, and transportation.
  • Respond to urgent participant requests and manage crisis response across multiple population types using appropriate clinical judgment, safety planning, and coordination with licensed mental health and crisis staff.
  • Participate in and help facilitate regular multidisciplinary case conferences with the Program Manager, Career Coach, and network of specialists.
  • Coordinate with re-entry case managers, probation officers, and legal service providers to support justice-involved participants' reintegration and workforce development goals.
  • Maintain accurate, timely, and comprehensive case notes and participant records in the organization's data management systems.
  • Ensure compliance with program, agency, and funder requirements related to client data, confidentiality, and documentation standards.
Desired Qualifications
  • A master's degree is strongly preferred.
  • Experience designing and facilitating group workshops or life skills sessions.
