Requirements
  • Experience in a community residence and/or nursing home is a plus.
  • Must be licensed by the New York State Department of Education.
  • CPR certification is preferred.
  • Must treat residents with respect and consideration.
Responsibilities
  • Manage nursing care for individuals, determine the need for immediate physician attention, execute physician or dentist medical orders, and coordinate nursing care with routine facility activities and individual treatment plans.
  • Oversee medical, dental, and nutritional care for residents with intellectual or developmental disabilities, adhere to applicable regulations, and review resident progress as appropriate.
  • Evaluate each client's nursing-care needs periodically and continuously, and provide care when appropriate under the supervision of a registered nurse and Qualified Intellectual Disabilities Professional.
  • Control communicable diseases and infection through identification and assessment, reporting to medical authorities, and implementing appropriate protective and preventive measures.
  • Develop a written nursing plan for each client requiring nursing care as part of the individual program plan.
  • Review each client's plan against daily needs at least quarterly, and more frequently for acute or chronic conditions requiring treatment or surveillance by a physician or other health professional; modify the nursing plan as necessary.
  • Monitor medication effects and periodically verify that clients take prescribed medications as needed, that medications are not outdated or discontinued, and that discontinued drugs are disposed of properly.
  • Periodically check that clients maintain adequate personal hygiene.
  • Secure physician services for client illness or injury and for continuing and preventive health services; arrange medical and dental appointments and hospital admissions.
  • Promptly inform the program manager or registered nurse of serious incidents or violations of resident or staff rights under established policies.
  • Use the nursing process to assess, plan, implement, and evaluate patient care; conduct focused interviews to identify patient needs and assess signs and symptoms of physiological and psychosocial changes.
  • Collect, analyze, and interpret data and information from records, and document actual or potential nursing diagnoses.
  • Record nursing observations and actions in medical records, including care, medications and treatments, treatment reactions, tests, fluid and food intake and output, vital signs, and nursing needs.
  • Work cooperatively with consulting and contracted medical services to ensure recommended treatments are carried out, integrated into treatment plans, and documented.
  • Maintain inventory standards for medicines, solutions, supplies, and equipment needed for nursing care; account for controlled-drug and narcotic issuance and inventories.
  • Protect residents' physical safety and security by assessing their ability to maintain their own safety and taking precautions against self-inflicted injury and environmental hazards, including measures to prevent infection transmission, isolate infected individuals, and use properly sterilized equipment when necessary.
  • Provide individuals and families with information about sustaining and preventive health care, personal hygiene, family or self-treatment methods, self-medication, and possible medication side effects.
  • Serve as a liaison, interpreter, or accompaniment for individuals receiving services from outside health providers.
  • Train direct support professionals through demonstrations and written or oral instructions on nursing techniques and use of nursing equipment, as needed.
  • Carry out agency management policies in accordance with OPWDD and/or other funding agency guidelines.
