Requirements
  • Associate Degree of Nursing from a college or university accredited by the National League for Nursing Accrediting Commission or the Commission on Collegiate Nursing Education.
  • Current, full, active, and unrestricted license to practice as a Registered Nurse.
  • One of the following certifications: Certified Case Manager, Certified Disability Management Specialist, Certified Rehabilitation Registered Nurse, Certified Occupational Health Nurse, Certified Occupational Health Nurse-Specialist, Advanced Certification in Continuity of Care, Certified Rehabilitation Counselor, Nurse Case Manager, or Care Manager Certified.
  • A minimum of three years of full-time clinical case management experience with diverse populations for candidates with an Associate Degree of Nursing, or two years for candidates with a Bachelor of Science in Nursing.
  • Two years of nursing experience after graduation.
  • Six to twelve months of case management experience.
  • Current American Heart Association Basic Life Support certification.
  • Additional certifications such as Advanced Cardiovascular Life Support, Pediatric Advanced Life Support, and Neonatal Resuscitation Certification as required by AFI 44-102 or equivalent.
Responsibilities
  • Provide expert case management for military members, veterans, and their families across various healthcare settings, focusing on acute, chronic, and complex health issues.
  • Advocate for military members by addressing their physical, emotional, and psychosocial needs and ensuring they receive appropriate care.
  • Coordinate and integrate care across interdisciplinary healthcare teams, levels of care, and sites.
  • Engage with the military community and foster relationships with service members and their families.
  • Contribute to the development and implementation of healthcare solutions, including telehealth initiatives, to improve accessibility and quality of care.
Desired Qualifications
  • Familiarity with the military healthcare system.
  • Telehealth experience.
  • Work experience with TRICARE and the Military Health System Genesis system.
