  • A Bachelor of Arts degree in public health, social work, health administration, or a related field.
  • Experience working in a health setting and two years of experience in a lead position or handling progressive administrative responsibility, or an equivalent combination of education and experience.
  • Ability to speak, read, and write an Asian language with high proficiency.
  • Proficiency with Microsoft Office applications, including intermediate to advanced competency in Excel and PowerPoint.
  • Ability to work collaboratively with AHS supervisors and managers, focusing on customer service and quality of care and exercising discretion, diplomacy, and tact in patient and staff interactions.
  • Communication skills sufficient to understand patients' and providers' or supervisors' instructions and accurately document patients' medical information; ability to communicate effectively with patients and staff with diplomacy and tact.
  • Basic working knowledge of medical terminology.
  • Facility and adaptability in working with computers and software applications.
  • Leadership skills and ability to train and guide staff in their duties.
  • Ability to perform moderate physical activity, including sitting, traveling, walking, driving, bending, lifting, and computer usage.

Responsibilities
  • Manage all aspects of the CLU program, including oversight of staff, grants and contracts, budget, patient and community education, psychosocial service referrals, patient care linkage, data collection and management collaboration with the Quality Management department, and patient advocacy.
  • Act as liaison between the CLU program and AHS departments and clinic locations; train and supervise CLU staff and other staff as appropriate; and represent the program to outside agencies.
  • Work with the Chief Medical Officer and provider leaders on health improvement and infectious diseases, including COVID-19, to plan, prioritize, and create mechanisms to improve patient care and community outreach and engagement.
  • Oversee and train CLU staff on program activities, including patient eligibility, registration, scheduling, data collection and documentation, risk assessment, referrals, insurance billing, and advocacy for patients and families.
  • Conduct outreach, advocacy, and education for the Asian community, emphasizing health issues and social factors influencing health.
  • Collaborate with the Chief Medical Officer, Specialty Mental Health Director, providers, and on-site specialists to provide support services, counseling, and patient education, including case coordination, medication-compliance education, brief psychosocial assessment, referrals, follow-up for high-risk and special-needs patients, and other program support services.
  • Collaborate with the Quality Improvement team to assess patient-care processes and outcome metrics and support high-quality care.
  • Liaise with other departments on health issues, including medical practitioners, patient education, eligibility, billing, and fiscal departments.
  • Represent the agency at meetings with service providers, including the Alameda County Department of Public Health.
  • Support grant-related plans, Patient Leadership Councils, Patient General Meetings, the Patient Newsletter, the Language and Culture Access Program, and patient education materials in partnership with clinic leadership and community leaders.
  • Maintain and annually update service protocols; enter CLU service data into the database; and generate reports for quality management, providers, and contract and grant requirements.
  • Develop and implement an organizational strategic plan to deliver programs and services that meet community needs.
  • Schedule and facilitate regular program meetings and case conferences as appropriate.
  • Serve as primary supervisor of CLU Program Coordinators and other staff; oversee recruitment, hiring, dismissals, disciplinary actions, evaluations, and other personnel actions in consultation with the Director of Community Services.
  • Consult and communicate with the Human Resources Manager to ensure compliance with collective bargaining agreements, personnel policies, and labor laws.
  • Develop staffing plans, assign work, and coordinate with other departments and Program Coordinators.
  • Ensure initial and ongoing training for program staff, clinic staff, and others who may serve at-risk or infected patients and community members.
  • Promote respectful and professional interactions with at-risk patients and community members.
  • Promote teamwork, cooperation, and positive workplace practices among staff and establish working relationships with other managers and staff.
  • Foster an environment that promotes trust and cooperation among clients and staff.
  • Enforce clinic policies and procedures, including HIPAA privacy requirements, to implement AHS principles.
  • Participate in planning organization-wide activities, including general membership meetings, fundraisers, special clinic days, and other events.
  • Participate in membership meetings, fundraisers, public events, outreach, agency advocacy, and ad hoc committees as required.
  • Attend AHS staff retreats and Board of Directors meetings as required.
  • Participate in agency-wide quality improvement processes and performance improvement teams.
