Requirements
  • A high school diploma, General Educational Development certificate, or Associate degree in a related field, plus two years of qualifying experience.
  • Ability to work in the field and/or Homebase office up to five days per week.
  • Ability to travel to housing court and Human Resources Administration/Department of Social Services offices up to two days per week.
  • Proficiency in database and client record system data entry and Microsoft 365.
Responsibilities
  • Oversee case management for program participants and their families by arranging, coordinating, monitoring, and delivering services related to their housing needs.
  • Conduct community follow-up to engage program participants in care, promote appointment compliance, and encourage self-sufficiency and empowerment.
  • Participate in outreach and engagement activities at Manhattan housing court and in the field, including Human Resources Recovery/Department of Social Services offices.
  • Promote linkages with service providers, especially legal aid providers, to help participants obtain and maintain housing stability and support housing court cases.
  • Monitor the effectiveness of linkages with other service providers.
  • Complete timely intakes, assessments, service plans, and referrals.
  • Provide service descriptions and overviews to prepare participants for full-service engagement.
  • Collaborate with program participants to develop service plans addressing barriers to housing stability.
  • Monitor and evaluate program participants' progress.
  • Prepare and submit Financial Assistance Requests on behalf of program participants.
  • Participate in developing after-care and ongoing housing assistance service-planning processes.
  • Coordinate supportive services and referrals based on participants' specific needs.
  • Assist participants with obtaining and reinstating benefits.
  • Assist participants in identifying financial resources and income supports.
  • Maintain participant documents in case files.
  • Monitor case progress and enter information into the CARES database when necessary.
  • Participate in and provide case-related updates during case management and general staff meetings.
  • Attend training and provide feedback on presentations.
  • Assist participants with obtaining employment services, including job training, job-readiness, and employment workshops.
  • Advocate for participants to receive work-support services such as rental assistance, food stamps, Medicaid, and subsidized childcare.
  • Assist participants with understanding leases, securing utilities, and making moving arrangements.
  • Conduct outreach to property owners and mediate issues that help participants locate or retain housing.
Desired Qualifications
  • A bachelor's degree in a related field.
  • Bilingual English/Spanish speaking ability.
  • Knowledge of the Department of Homeless Services, the Human Resources Administration, and entitlements.
  • Knowledge of the complex interaction of homelessness, incarceration, substance abuse, mental health, and domestic violence, with sensitivity to cultural and gender-specific issues affecting individuals and families.
