Responsibilities
  • Provide psychosocial services to in-center and home dialysis patients, as applicable, using social work theory and accepted social work practice methods.
  • Work with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for patients.
  • Inform, educate, and support staff regarding the emotional, psychological, and behavioral impact of chronic kidney disease on patients and families.
  • Participate in comprehensive patient assessments by evaluating psychosocial status, strengths, and areas of need that may affect rehabilitation and treatment outcomes.
  • Participate in care planning with patients and the healthcare team to identify interventions supporting rehabilitation, treatment goals, and quality of life.
  • Use patient education programs, social work theory and methods, social work-focused interventions, and quality-of-life measurement instruments during assessment and care planning.
  • Monitor and intervene to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life.
  • Provide supportive counseling within the scope of clinical training and state licensure.
  • Provide educational and goal-directed counseling to patients seeking transplantation.
  • Provide information and assist patients and the care team with referrals to community resources, including home health services and vocational rehabilitation.
  • Maintain current knowledge of local vocational and educational rehabilitation programs and assist patients with referrals and access.
  • Assess patient awareness of advance directives and assist with forms, information, and discussions of advance-directive wishes when necessary.
  • Participate with the physician and nurse in discussions of patient do-not-resuscitate status to support informed decisions.
  • Follow social work and patient physical and mental function policies, including documentation requirements.
  • Document social work interactions and interventions provided to patients and families.
  • Provide psychosocial support and social work-focused interventions addressing non-adherence, quality outcomes, and quality-of-life concerns.
  • Participate in monthly quality review meetings and report adherence-related quality indicators, including missed and shortened treatments, quality-of-life trends, and service recovery.
  • Assess patient knowledge of kidney disease and identify barriers affecting treatment adherence.
  • Provide education tailored to each patient's learning style, communication barriers, and needs.
  • Provide information about treatment modalities with the interdisciplinary team.
  • Facilitate the transplant referral process and collaborate on transplant waitlist management.
  • Educate patients and families about psychosocial issues related to end-stage renal disease and available support services.
  • Review patient rights, responsibilities, grievance information, and facility policies with patients or their representatives.
  • Collaborate with the team on quality assessment and improvement activities.
  • Review admission-related patient rights, responsibilities, grievance procedures, non-discrimination policies, do-not-resuscitate statements, and immediate needs or concerns.
  • Support the referral and admission process and help schedule patients to maximize adherence and adjustment.
  • Interview patients requesting discharge to identify root causes or concerns and share them with operational leadership.
  • Collaborate with insurance coordinators as a patient liaison on insurance issues.
  • Provide information and education about dialysis payment through federal, state, commercial, renal, assistance, and entitlement programs.
  • Coordinate with insurance staff regarding patient-status changes affecting insurance coverage.
  • Refer patients to the patient billing solutions department for treatment-bill questions and concerns.
  • Work with administrative support staff to maintain current patient resource lists, including transportation resources.
  • Provide staff training on psychosocial topics as needed.
  • Participate in weekly team huddles and discuss urgent patient issues with staff.
  • Perform other related duties as assigned.
