Requirements
  • A Doctor of Pharmacy doctorate or registered pharmacist qualification is required.
  • Knowledge of pharmacokinetic principles is required for medication dosing.
  • The pharmacist must monitor patients for response to medications.
  • The pharmacist must identify and document sentinel or serious events, near misses, medication errors, and adverse drug reactions in the electronic reporting system.
  • The pharmacist must promote and assure compliance with the hospital formulary and policies and procedures defined by the Pharmacy and Therapeutics Committee.
  • The pharmacist must provide care appropriate to the chronological age, developmental age, social maturity, and cultural background of the patient.
  • A Pennsylvania registered pharmacist license or a New Jersey registered pharmacist license is required.
  • Certification is required based on the primary work location.
Responsibilities
  • The Clinical Pharmacist oversees the medication distribution system for their area of responsibility.
  • The Clinical Pharmacist identifies, prevents, and resolves drug therapy problems by ensuring each medication is indicated, effective, safe, and convenient for the patient.
  • The Clinical Pharmacist interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • The Clinical Pharmacist assures the appropriateness of drug therapy and compliance with hospital policies.
  • The Clinical Pharmacist oversees the distribution of medications throughout Thomas Jefferson University Hospital.
  • The Clinical Pharmacist supports implementation of processes to assure the quality of Pharmacy services.
  • The Clinical Pharmacist trains staff as assigned, including residents, interns, technicians, and new pharmacists.
  • The Clinical Pharmacist precepts student pharmacists as assigned in accordance with the Preceptor Development and Evaluation Program policy.
Desired Qualifications
  • One year of PGY1 Pharmacy Residency is preferred.
  • Microsoft Word and Microsoft Outlook skills are preferred.
