Requirements
  • A high school diploma or General Educational Development credential is required.
  • Current cardiopulmonary resuscitation certification is required.
  • Strong interest in working in a health care setting is required.
  • Excellent verbal and written communication skills are required.
  • The employee must be able to stand for prolonged periods, bend over, and lift or lower solutions and equipment as specified.
  • The employee must complete the Ishihara Color Blindness test as a condition of employment.
  • The employee must comply with confidentiality requirements, company policies, and applicable laws and regulations.
Responsibilities
  • Greet new and existing patients and notify the Patient Care Team of their arrival.
  • Schedule appointments and place appointment reminder calls, including appointments for access placement and removal.
  • Answer home therapy program calls and follow up on voicemails with the applicable care team member.
  • Coordinate transportation assistance as needed.
  • Prepare rooms and spaces for patient arrival in accordance with infection control standards.
  • Place supply orders and follow up on patient-related orders with customer service.
  • Prepare education materials and laboratory supplies for each patient.
  • Track extension set changes, vaccine due dates, hospitalizations, transplant referrals, and required follow-up.
  • Ensure patient records are available for review.
  • Assist patients to and from the waiting area during appointments.
  • Obtain patients’ height, weight, and vital signs.
  • Perform venipunctures only for laboratory testing.
  • Process laboratory samples.
  • Follow up on referrals and additional appointments resulting from patient appointments.
  • Document patient information as applicable.
  • File and upload records and store documents.
  • Track transfer set changes, Kt/Vs, CIA, point-of-care home visits, vaccines, hospitalizations, infections, and monthly education.
  • Track physician and employee certifications, licensures, and annual skills.
  • Complete the medical record retention checklist.
  • Collect and track transplantation laboratory materials.
  • Check for expired medications, laboratory tubes, and supplies.
  • Count and order nonprescription supplies and assist with ordering supplies, including medication.
  • Clean and stock assigned areas for future patient appointments.
  • Maintain confidentiality of patient information and medical records.
  • Participate as part of the Care Team Hub and perform other assigned duties.
  • Travel to other local home facilities in the region when required.
Desired Qualifications
  • Zero to six months of experience is preferred.
  • Prior experience in a physician office or hospital is preferred.
  • Proficiency in the Microsoft Office Suite is preferred.
