- Construct personalized clinical care plans and define actionable wellness milestones.
- Monitor and manage long-term health conditions to ensure stability and prevent nighttime complications.
- Administer post-operative recovery care, wound management, and medication.
- Educate clients and their designated representatives about medical diagnoses and daily care routines.
- Provide overnight support to ensure patient comfort, safety, and well-being.
- Home Health experience is preferred.
