  • Board certification or board eligibility in Internal Medicine.
  • An active, full, and unrestricted license to practice medicine in any United States state or territory.
  • An active National Provider Identifier report.
  • An active Drug Enforcement Administration license.
  • Ability to pass a federal background investigation.
  • Ability to sit, stand, walk, lift, squat, bend, twist, and reach above the shoulders during the work shift.
  • Ability to lift up to 50 pounds from floor to waist.
  • Ability to lift up to 20 pounds.
  • Ability to carry up to 40 pounds a reasonable distance.
  • Ability to push or pull with 30 pounds of force.
Responsibilities
  • Provide primary care within the scope of practice and privileging.
  • Provide comprehensive primary care services, including women’s health, to a defined panel of patients.
  • Work with clinic staff to schedule new and follow-up patients.
  • Take histories and perform physical examinations; order laboratory and diagnostic studies; diagnose acute and chronic conditions; and formulate treatment plans within the scope of practice and privileging.
  • Order and monitor appropriate medications using the VA Formulary; query state Prescription Drug Monitoring Programs before prescribing controlled substances; and obtain required IMED consents before prescribing or dispensing medications requiring consent.
  • Become familiar with the VA Formulary and use the computer system as needed for efficient practice.
  • Provide outpatient primary care across prevention, diagnosis, treatment, referral, and follow-up for simple to moderately complex conditions, including chronic disease, mental health, women’s health, preventive screening, and acute care.
  • Refer patients to specialty care, admit patients to hospital care when indicated, and provide patient education and counseling.
  • Complete patient documentation promptly in the CPRS system and VA electronic medical record in accordance with CMS evaluation and management documentation guidelines.
  • Maintain patient problem lists, update diagnoses, medications, and procedures as patient conditions change, and complete clinical reminders in the electronic medical record.
  • Ensure patient care plans contain clinically indicated recommendations and arrange or provide consented care for patients assigned to the PACT.
  • Provide leadership to the care team by delegating appropriate care processes, reviewing clinical and performance data, and improving critical team processes.
  • Ensure same-day access for face-to-face and telephone care visits during regular clinic hours.
  • Collaborate with PACT staff to develop personal health plans incorporating appropriate care management and care coordination.
  • Consult with and instruct referring physicians regarding appropriate procedure indications and clinically appropriate work-ups.
  • Determine appropriate treatment courses and communicate in person or by phone with referring clinicians.
  • Initiate appropriate social work referrals for identified homeless patients.
  • Follow established medication policies and procedures and do not provide sample medications to patients.
