Requirements
  • A Bachelor's degree or Associate's degree in Nursing is required, along with New York licensure.
  • Current CPR and First Aid certification must be maintained.
  • A successful completion of an LPN license program is required.
  • The role requires the ability to undertake diverse care tasks and work in a multidisciplinary team.
  • The candidate must be familiar with hospital health, safety, and sanitation standards and procedures.
  • The candidate must understand confidentiality obligations and nursing best practices.
  • The candidate must have knowledge of medical and hospital terminology.
  • The candidate must have knowledge of Microsoft Office and data entry.
  • The candidate must have the physical condition and stamina to perform the role.
  • The candidate must be able to handle stress and apply time-management skills to meet deadlines and prioritize workload.
Responsibilities
  • Plan and manage client care according to each individual's needs.
  • Interview clients and record their medical history, physical conditions, and vital signs.
  • Support the Registered Nurse with field visits, administering injections, medical education, oral medication delivery, and medication-room monitoring on Assertive Community Treatment teams.
  • Assess, monitor, and ensure access to coordinated medical and mental health services.
  • Administer intramuscular injections and supply medications to consumers as prescribed by a psychiatrist.
  • Meet with consumers in the community on an ongoing basis to monitor health and assess progression or regression in behavior or appearance.
  • Meet more frequently with consumers who have unstable medical or mental health status.
  • Assist consumers with filling prescriptions and coordinating appointments.
  • Refer consumers to emergency care when needed and coordinate emergency treatment with emergency responders and emergency-room staff.
  • Coordinate treatment and discharge planning with hospital staff.
  • Meet with consumers in person and through telehealth visits.
  • Provide feedback and support to staff during morning meetings regarding client needs and issues.
  • Remain available by cell phone for emergency consultation and crisis intervention.
  • Complete progress notes in a timely manner.
  • Maintain nursing and health-related data entry in the electronic health record.
  • Assess medical-care needs and other services that support wellness.
  • Advocate and act as a liaison for recipients securing medical care and community supports that promote integrated physical and mental health wellness.
  • Review and audit medication-management procedures.
  • Transcribe orders and update Medication Administration Records.
  • Monitor vital signs and medication side effects and report findings to the team.
  • Contribute to medication-management procedures with the Registered Nurse and program psychiatrist.
  • Assist the Registered Nurse and psychiatrist in coordinating medical care.
  • Support the Registered Nurse in ensuring consumers have primary-care providers and receive annual physical and dental examinations.
  • Provide individual health counseling as needed.
  • Comply with employee, corporate compliance, and New York State Department of Social Services policies, procedures, rules, regulations, and statutes.
  • Maintain accurate, complete, and timely consumer chart documentation.
  • Participate in team meetings, in-service training, and required online training.
  • Assist the full-time Registered Nurse.
  • Maintain records according to agency regulations.
  • Maintain relationships with outpatient and housing programs and build relationships with organizations that can assist consumers who need additional services.
