Responsibilities
  • Deliver program activities under the direction of the community outreach and housing director and corps officers to ensure contract compliance and adequate standards of care for residents.
  • Deliver case management services to residents in the assigned supportive permanent housing units, tailoring support frequency to resident needs.
  • Meet with residents on an ongoing basis based on client needs.
  • Coordinate monthly life-skills workshops with subcontractors and provide individual life-skills assistance as needed.
  • Coordinate with subcontractors to develop individualized service plans for each resident based on needs, interests, and skill levels.
  • Identify client needs and make referrals to community providers.
  • Provide empathetic, non-judgmental support during crises and ongoing case management; monitor and manage client agreements and arrange reasonable accommodations when needed.
  • Monitor client progress toward individualized goals.
  • Participate in quarterly client case reviews and provide case consultation to other staff regarding client needs.
  • Collaborate with external partners, community providers, and agency staff to support residents and ensure quality of care.
  • Maintain service statistics, case records, client confidentiality documentation, assessments, service plans, referrals, termination records, and eviction documentation using database tools.
  • Network with community providers, advocate for clients, and represent the program and agency at community meetings and case conferences.
  • Assess the immediate and urgent needs of families and individuals, listen to client concerns, and connect clients with appropriate social-services programs.
  • Conduct intake, evaluations, and assessments of prospective clients referred through partner agencies.
  • Develop individualized service plans in accordance with agency and funding-source goals and objectives.
  • Maintain statistical accountability and fulfill contractual obligations for The Salvation Army and its funding sources.
  • Assist with outreach activities and collaborate with other agency programs.
  • Develop action plans addressing clients' root causes and needs, set goals and outcomes, and identify resources to meet those goals.
  • Establish collaborations with outside agencies to advocate for clients and strengthen their ability to make informed decisions.
  • Consult with external agencies to provide support services and resources.
  • Provide brokerage, networking, advocacy, and referrals to youth service agencies and programs.
  • Provide individual counseling sessions and conduct home and agency visits.
  • Provide counseling and contact with family members when appropriate.
  • Stay in touch with clients after referrals to ensure services were beneficial and needs remain met.
  • Maintain confidentiality, respect privacy, and preserve clients' routines and independence as much as possible.
  • Recognize clients' spiritual needs and refer them to corps officers for pastoral care when appropriate.
  • Coordinate services with seasonal social-service programs as requested by corps officers and divisional headquarters.
  • Answer basic caller questions or make referrals regarding The Salvation Army and community-based programs.
  • Share matters affecting the overall welfare of corps social services with the Community Outreach Director and Corps Commanding Officer.
  • Prepare monthly statistics for staff meetings.
  • Prepare Thrift Store Assistance Vouchers when necessary.
  • Respond to correspondence concerning social services.
  • Assist with public-relations events and campaigns as assigned.
  • Complete other duties assigned by the Commanding Officer or based on operational needs.
  • Participate in required social-service, homelessness-counseling, supportive-housing casework, and other appropriate Salvation Army training seminars.
