Requirements
  • A high school diploma or equivalent is required.
  • At least 2 years of experience in community health, social services, or a related field is required.
  • Knowledge of behavioral health systems, local resources, and social determinants of health is required.
  • Strong communication, cultural competency, and interpersonal skills are required.
  • Ability to engage clients and families effectively and provide supportive services is required.
  • Organizational skills to manage caseloads and documentation are required.
  • Proficiency with technology for case documentation and communication is required.
  • Ability to travel locally within the community is required.
  • A valid California driver’s license and reliable transportation are required.
  • Comfort working in a field-based environment is required.
  • Successful completion of a background check and submission of required documentation and new-hire paperwork are required.
Responsibilities
  • Provide services in the community, at Full Service Partnership sites, and at the Santa Rosa office.
  • Act as an empathetic guide and non-clinical mentor to help Full Service Partnership clients and their families navigate health and social service systems.
  • Facilitate educational workshops and weekly drop-in Resource Clinics.
  • Collaborate directly with Full Service Partnership teams, clients, and their support networks.
  • Travel locally within Sonoma County to meet members and community partners.
  • Engage families and coordinate directly with the client and their Full Service Partnership team to develop a family-inclusive Individual Service Plan within 30 days of enrollment.
  • Provide guidance on accessing all levels of care within Sonoma County Department of Health Services Behavioral Health Division, including Specialty Mental Health Services and Drug Medi-Cal Organized Delivery System services.
  • Connect families to housing assistance, food security programs, and employment services through closed-loop referrals.
  • Provide information, advocacy, and direct support regarding legislation, legal processes, and formal grievance processes through the Department of Health Services Behavioral Health Division Patients’ Rights Advocate.
  • Host educational workshops on effective communication strategies, crisis de-escalation, boundary setting, and caregiver self-care.
  • Provide psychoeducation on serious mental illness and substance use disorder diagnoses, recovery models, and harm reduction principles.
  • Staff weekly drop-in Resource Clinics at community-based locations using a “No Wrong Door” approach to help the public access social services.
  • Conduct targeted outreach to local community centers, faith-based organizations, and nonprofits to distribute culturally and linguistically appropriate information.
  • Lead community education and social media initiatives to reduce stigma and foster understanding of behavioral health challenges.
  • Connect families with local support groups, such as NAMI Sonoma County and Al-Anon, to foster peer support and resilience.
  • Track encounters, referrals, demographics, and family-inclusive Individual Service Plan documentation using the designated electronic health record system.
  • Record attendance for educational workshops and Resource Clinics.
  • Gather qualitative feedback through post-workshop surveys to measure family empowerment and cultural responsiveness.

  • Shared lived experience of program families is highly prioritized.
