About the job
- A bachelor's degree from an accredited institution in a human services field is required.
- At least one year of post-bachelor's experience in a behavioral health setting is required.
- Completion of the approved 12-hour Targeted Case Management Core Training is required.
- Completion of the approved six-hour Substance Use Disorder Targeted Case Management Training is required.
- Completion of the approved six-hour Severe Emotional Disability Targeted Case Management Training is required.
- Completion of the approved six-hour Severe Mental Illness Targeted Case Management Training is required.
- The candidate must be at least 21 years of age.
- A valid driver's license, reliable transportation, and proof of current auto insurance are required.
- The candidate must submit to required background checks and pre-contract drug screening.
- The candidate must effectively use agency communication systems.
- The candidate must have strong interpersonal and client-engagement skills.
- The candidate must demonstrate professional maturity and respect for others.
- The candidate must have strong organizational and time-management skills.
- The candidate must be able to work with minimal supervision after completing training.
- The candidate must follow through with documentation, communication, and deadlines.

- Assess the needs of clients and their families.
- Link clients and families to appropriate resources.
- Coordinate appropriate community resources and services.
- Manage an assigned caseload.
- Respond appropriately to client needs in compliance with Medicaid standards.
- Develop, implement, and review client service plans.
- Work with other community resources to achieve client service needs.
- Serve clients with diverse populations and varied clinical needs.
