Requirements
  • A bachelor's degree in public health, community health, or social work, or an associate degree in public/community health or a healthcare-related field with at least one year of directly related or community-based experience.
  • Completion of South Carolina Community Health Worker Certification within one year of hire and/or another Community Health Worker training course.
  • A valid driver's license, an acceptable three-year motor vehicle record, and a certificate of auto-insurance.
  • Good organizational and time-management skills.
  • Ability to work independently with accountability and exercise sound judgment, discretion, and professionalism.
  • Availability to work occasional evenings and weekends.
  • Ability to travel and independently meet with patients in clinic, home, or community-based settings.
  • Ability to perform the stated physical, sensory, and environmental job functions, including standing, walking, lifting up to 15 pounds, maintaining corrected 20/40 vision and hearing acuity, performing frequent fine motor movements, working in a latex-safe environment, and qualifying physically for respirator use.
Responsibilities
  • Collaborate with patients, family members and caregivers, healthcare professionals, and community-based organizations to address social care needs and socioeconomic barriers to health.
  • Conduct screenings and assessments for Palliative Care and Geriatrics patients living with serious illness to identify resources for unmet social, medical, and behavioral needs and facilitate assistance or interventions between care visits.
  • Provide education and information about available community resources and services to Palliative Care and Geriatrics patients and their family members or caregivers.
  • Build relationships with Palliative Care and Geriatrics patients and community-based organizations, and follow up to close the loop and ensure needed services are received.
  • Assist high-risk patients with social care program enrollment forms and applications, and follow up to close the loop and ensure needed services are received.
  • Gather information, document it, enter data into the electronic medical record and social care electronic platforms, and contribute to data analysis.
  • Summarize progress to the leadership team regularly to communicate program success and engage leadership in quality initiatives and improvement processes.
