About the job
- An active New Jersey Registered Nurse license is required.
- Current CPR/BLS certification is required.
- Strong clinical assessment and communication skills are required.
- Must be reliable, compassionate, and professional.
- Must be comfortable working independently in a home-care setting.
- Provide skilled nursing care based on each client’s individualized care plan.
- Assess and monitor clients’ health conditions and clinical needs.
- Administer medications and prescribed treatments.
- Perform wound care, catheter care, and other skilled nursing procedures within the scope of practice.
- Recognize and report changes in the client’s condition.
- Coordinate care with physicians, families, and other healthcare team members.
- Maintain accurate and timely clinical documentation.
- Educate clients and families regarding medications, treatments, and care plans.
- Promote the client’s comfort, safety, dignity, and independence.
- Provide professional and compassionate care in the client’s home.
- One or more years of nursing experience is preferred.
- A valid driver’s license and reliable transportation are preferred.
- EHR and clinical documentation experience are preferred.
