Requirements
  • Current Registered Nurse or Registered Practical Nurse registration with the College of Nurses of Ontario, with authorization to practice without restrictions.
  • At least 5 years of experience practicing as a Registered Nurse or Registered Practical Nurse.
  • Comprehensive patient health assessment skills.
  • Understanding of community nursing practice principles.
  • Experience leading and managing a team.
  • Knowledge of clinical documentation standards and practice guidelines.
  • Organizational, interpersonal, and teaching skills.
  • Commitment to professional development relevant to home health care clinical practice.
  • Critical-thinking and problem-solving skills.
  • A valid driver's license and access to reliable transportation.
  • A current, clear Vulnerable Sector Screening.
  • A Community Health Nurse Certification from the Canadian Nurses Association is considered an asset.
Responsibilities
  • Supervise and support personal support workers and home support workers to ensure practices comply with legislated standards of practice and company policies and procedures.
  • Respond to and advise on or resolve concerns from community staff, patients and families, and other team members.
  • Provide ongoing leadership through regular huddles, face-to-face meetings, and one-on-one meetings.
  • Initiate patient safety incident investigations and document them to ensure timelines and action plans are met.
  • Collaborate with nursing team members to support clinical documentation and submissions according to specific funder reporting guidelines.
  • Engage patients in collaborative problem-solving, provide health teaching, build rapport and trust, and identify patient health care needs, strengths, capacities, and goals.
  • Engage staff in problem-solving and provide education, coaching, and mentoring to resolve problems.
  • Maintain the knowledge, skills, and attitudes required to provide safe, efficient, and effective service across all areas of practice by keeping current with changes affecting scope of practice.
Desired Qualifications
  • A Community Health Nurse Certification from the Canadian Nurses Association (CCHN(C)) is considered an asset.
