Requirements
  • An active, unrestricted Licensed Practical Nurse or Licensed Vocational Nurse license, with residency, in the state of service.
  • At least two years of experience as an LPN/LVN in a direct, patient-facing care-delivery role.
  • Current Basic Life Support or cardiopulmonary resuscitation certification for healthcare providers, or willingness to obtain it.
  • Demonstrated ability to perform high-quality venipuncture and capillary blood draws on geriatric patients in home settings where required by the market and consistent with state law.
  • A valid driver’s license with an acceptable driving record and proof of an active auto-insurance policy.
  • Strong communication skills to effectively interact with patients, families, and remote healthcare professionals.
  • Ability to work independently and make sound decisions while adhering to established protocols, guidelines, and the LPN/LVN scope of practice.
  • Organizational and time-management skills to balance in-person visits, telehealth facilitation, and administrative tasks.
  • Familiarity with electronic medical record systems and computer skills, including Microsoft Word, Excel, and PowerPoint.
  • Understanding of risk assessment, quality measures, and full-risk or value-based care.
  • Experience working with culturally diverse and underserved populations.
  • Ability to stand or sit for extended periods, with visual acuity and fine motor skills.
  • Ability to travel to patient homes and facilities as needed.
  • Ability to operate and drive a personal vehicle for company business safely and professionally.
  • Ability to maintain required licensure and certifications and provide valid copies before expiration.
  • Legal authorization to work in the United States without current or future employer sponsorship.
  • Ability to perform safety-sensitive duties with full cognitive and physical functioning and comply with applicable drug and alcohol testing and marijuana prohibition requirements.
Responsibilities
  • Facilitate in-home telehealth visits between the WellBe advanced practice provider or physician and patients, including initial assessments, urgent visits, and follow-up visits.
  • Check and record weight, blood pressure, heart rate, lung sounds, and blood-sugar levels, and document health changes observed or reported by patients.
  • Complete and document thorough home-safety inspections during patient visits.
  • Provide direct patient-care support and patient education under the direction of the assigned provider within the LPN scope of practice.
  • Complete minimally invasive patient screenings and point-of-care testing and explain the testing to geriatric patients.
  • Perform venipuncture and capillary blood collection for geriatric patients in home settings when permitted by state law, training, and company protocols.
  • Perform intravenous therapy when permitted by state law, training, and company protocols.
  • Support comprehensive initial, follow-up, and urgent visits; complete drop-in visits for unengaged and disengaged patients; and support open HEDIS measures.
  • Conduct check-in visits and calls with lower-acuity patients to support the “call us first” model.
  • Escalate clinical concerns to the advanced practice provider, Community Operations Director, or Community Medical Director.
  • Develop rapport, deliver compassionate in-home service, provide customer service, and treat patients and staff respectfully.
  • Act as a community resource and engage patients who are not currently engaged in care.
  • Schedule patient appointments with the WellBe team and coordinate specialist appointments and diagnostic tests.
  • Answer patients’ clinical questions within the boundaries of standard operating procedures.
  • Explain WellBe processes and the care journey.
  • Review patient-care interventions and assess needs related to transitions of care, end-of-life discussions, palliative-care interventions, and medication adherence.
  • Support the advanced practice provider in panel management and quality-of-care efforts; use HEDIS tracking tools to improve HEDIS and STARS performance and close gaps, including medication-adherence gaps.
  • Enter complete, accurate, and timely data into patient records and the electronic medical record and submit required documentation daily.
  • Collaborate with the interdisciplinary team and participate in scheduled team and Interdisciplinary Team meetings, providing information about patient and family interactions.
  • Build relationships with local community providers and caregivers to maintain the company’s presence and reputation in local markets.
  • Attend required training, including equipment and device training, and complete ongoing education as skills and protocols are added.
  • Travel locally to patients across homes, hospitals, skilled nursing facilities, and assisted-living settings.
  • Complete other tasks needed to accomplish the team’s objectives and goals.
