  • Valid New Mexico driver's license, liability insurance, insurability under the agency insurance carrier, dependable vehicle, and clean driving record.
  • Ability to acquire CPR and First Aid certification.
Responsibilities
  • Contact participants and provide case management services to an assigned caseload according to program guidelines and scope of work.
  • Travel as required based on caseload and scope of work.
  • Orient participants to organizational policies and procedures, including rights and responsibilities, programs and services, grievance procedures, and confidentiality requirements.
  • Provide case management and refer participants to support services such as medical evaluation and treatment and social services.
  • Build and maintain collaborative relationships with community agencies.
  • Maintain accurate and current case records and document participant contacts in a timely manner.
  • Complete required documentation, including case notes, program documentation, and required reports, within designated timeframes.
  • Review and submit participant budgets, reimbursements, and other program documentation required by the program contract.
  • Complete, verify, and submit monthly billing within established timeframes.
  • Close inactive cases monthly to maintain an accurate caseload.
  • Attend staff meetings, provide updates on participants with complex issues, and receive new referrals.
  • Maintain practices in accordance with applicable laws, regulations, professional standards, contract guidelines, and program requirements.
  • Maintain HIPAA compliance.
  • Ensure the safety of persons served under the role's supervision.
  • Implement CARF standards and follow professional ethics under the Code of Conduct and Business Ethics policy.
  • Maintain the confidentiality of privileged information.
  • Participate in educational programs, current-literature review, in-service activities, meetings, and workshops to support professional growth.
Desired Qualifications
  • One to three years of work experience in case management or social work.
  • Certified Case Manager, Certified Rehabilitation Counselor, or Master of Social Work qualification.
  • Bilingual ability.
  • Bachelor's degree in social services or a related human services field.
