Requirements
  • A Pharm.D. or Bachelor of Pharmacy degree from an accredited school of pharmacy.
  • No prior experience is required.
  • Basic Life Support Healthcare Provider certification is required within 90 days of employment; other resuscitation certifications may be required depending on the department.
  • A pharmacist license from the applicable state board or department of professional regulation is required for the state of work location.
  • Ability to perform frequent lifting, carrying, pushing, pulling, sitting, standing, walking, reaching, and repetitive foot, leg, hand, and arm movements.
  • Ability to perform occasional bending, stooping, kneeling, squatting, twisting, and gripping, occasional handling of objects weighing 25–50 pounds, and rare climbing.
  • Frequent use of vision and depth perception, hearing and speech for oral communication, and keyboard/data-entry skills.
Responsibilities
  • Gather necessary data to promote patient safety by ensuring that each relevant medication is indicated, effective, safe, and that the patient is adherent to therapy.
  • Improve patient-specific drug therapy and therapeutic problems by evaluating the appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects.
  • Assess relevant patient-specific medical problems, prioritize them based on urgency and severity, identify preventive and health-maintenance issues, and design and justify drug-therapy plans when appropriate.
  • Provide enhanced patient care through medication and disease-state counseling, adherence assessment, medication-cost analysis, immunization-status review, and communication with patients, providers, and pharmacies.
  • Provide consultation to other health care professionals when requested.
  • Conduct patient counseling and monitoring as required by the specialty-pharmacy patient-management program.
  • Participate in the specialty-pharmacy quality-management program and continuous process-improvement and quality-improvement projects.
  • Precept pharmacy students and residents, as applicable.
  • Work in a constant state of alertness and in a safe manner.
  • Perform other duties as assigned.

  • Oncology or infusion experience is preferred but not required.
