Requirements
  • A current pharmacist license issued by the Oregon State Board of Pharmacy.
  • OHSU may require training or experience in a specialized area at the time of recruitment.
Responsibilities
  • Assist with reconciliation of home medications upon admission and discharge, educate patients regarding home medication regimens before and after surgery, coordinate discharge medication plans with the interdisciplinary team, and document activities in the patient medical record.
  • Interpret and evaluate prescribers' orders for accuracy, completeness, and appropriateness, and document clinical decisions or medication distribution issues requiring follow-up or continued monitoring and evaluation.
  • Monitor therapeutic response and adverse medication effects, consult providers regarding identified questions or problems, document activities in the patient medical record, and report adverse drug reactions, drug defects, and medication errors.
  • Respond to drug-information requests from prescribers, nurses, patients, and patients' families by reviewing and interpreting literature and applying it to specific patient-care situations.
  • Identify, resolve, and document complex therapeutic problems using advanced knowledge for a specific patient population.
  • Assist in developing treatment guidelines for patients with specific diagnoses and document activities in the patient medical record, pharmacy monitoring profile, drug-use evaluation data collection, or adverse-drug-reaction reporting program.
  • Influence therapy standards through peer-level relationships with prescribers and serve as a medication authority in the practice area for prescribers, nurses, and pharmacists.
  • Participate in intra- and interdepartmental education programs and contribute to peer development.
  • Supervise technicians, students, interns, and residents; assume responsibility for their duties and monitor and evaluate their performance.
  • Verify the accuracy and appropriateness of medications prepared and dispensed from the pharmacy.
Desired Qualifications
  • A Bachelor of Science in Pharmacy or Doctor of Pharmacy degree plus a practice residency or equivalent.
  • A Postgraduate Year 2 specialty residency.
  • Basic Life Support or Advanced Cardiovascular Life Support certification.
  • Vaccination certification.
  • Teaching certificate.
  • Experience in medical/surgical pharmacy practice in inpatient or outpatient settings.
  • Experience with outpatient oncology and non-oncology treatment and infusion plans.
  • Inpatient hospital pharmacy practice serving acute care, psychiatric, rehabilitation, skilled nursing, long-term care, obstetric, oncology, chemically dependent, HIV, home care, and hospice patient populations.
  • Specialist certifications such as Board Certified Pharmacotherapy Specialist or Board Certified Oncology Pharmacist Specialist.
  • State of Oregon pharmacy preceptor licensure.
