Requirements
  • Certification as a Medication Aide or compliance with state medication-aide requirements.
  • At least one year of experience working with elders or disabled individuals is preferred.
  • Demonstrated leadership capabilities.
  • Knowledge and experience in assisted living, dementia care, or skilled nursing is preferred.
  • Ability to work weekends, evenings, and flexible hours during peak service-delivery periods.
Responsibilities
  • Identify residents’ ongoing needs and services through the assessment and Personalized Service Plan process.
  • Ensure proper follow-through and documentation when residents’ clinical care needs change.
  • Support the Wellness Nurse by completing the clinical sections of monthly wellness visits and Personalized Service Plans accurately.
  • Inform the Wellness Director or Executive Director of medication or service-level changes that may result in a higher tier level or potential move-out.
  • Assist with obtaining monthly weights and vital signs for each resident before the monthly wellness visit.
  • Assist with resident care as needed.
  • Provide medication-care assistance or administration, communicate with resident support services and families, and maintain clinical quality assurance while supporting wellness programs and services.
