Requirements
  • At least one year of provider experience is required.
  • The Nurse Practitioner must graduate from an accredited School of Nursing program, be licensed as a registered nurse, and be certified as a Nurse Practitioner through an accredited Nurse Practitioner Program with a Master's Degree in Nursing and a concentration and specialty track in Older Adult and/or Family Nurse Practitioner. Other concentrations or specialty tracks may be considered based on experience and business need.
  • The Physician Assistant pathway requires graduation from an ARC-PA-accredited program, completion of the PA National Certifying Exam to use the PAC designation, state licensure, and NCCPA certification.
  • An unrestricted professional license and DEA registration are required for each state in which the provider practices, where practice allows.
  • The position requires providing care in a skilled nursing and long-term care setting.
Responsibilities
  • Provide primary and urgent care services to nursing center residents in collaboration with other medical providers in the center.
  • Collaborate with the Director of Clinical Practice and/or Area Clinical Practice Manager.
  • Collaborate on-site with the Center Leadership Team on center-specific goals and priorities.
  • Collaborate with the Medical Director and/or Attending Physician on patient-specific medical care.
  • Work collaboratively with Center Leadership to improve patient outcomes through high-quality health care and compassion.
Desired Qualifications
  • Two years of experience working with adult or geriatric populations is preferred.
