Requirements
  • A nursing degree from an accredited college or university, or graduation from an approved licensed practical nurse/licensed vocational nurse program.
  • A current, unencumbered, active license to practice as a licensed practical nurse/licensed vocational nurse in the state.
  • Current cardiopulmonary resuscitation certification.
  • Knowledge and experience with PCC are preferred for the role.
  • Maintenance of all required continuing education and licensing.
  • Good standing with the State Board of Nursing at all times.
  • Ability to read, write, speak, and understand English, including technical procedures and policy and procedure manuals.

Responsibilities
  • Direct the day-to-day functions of nursing assistants in accordance with rules, regulations, and guidelines governing the long-term care facility.
  • Ensure nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Review policies, procedure manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total regimen of care.
  • Participate in the nursing service quality assurance program and facility surveys.
  • Review, update, and assist with the nursing-services portion of residents’ discharge plans.
  • Admit, transfer, and discharge residents; complete related forms and documentation.
  • Complete accident and incident reports and chart related events.
  • Maintain resident census reports and forward charge slips and required information to the Business Office.
  • Complete medical forms, reports, evaluations, studies, charting, and other administrative records.
  • Protect resident health information, user credentials, and facility information systems, and report suspected violations or unauthorized access attempts.
  • Complete and file records and charts for resident admissions, transfers, and discharges.
  • Receive and record physician telephone orders and transcribe orders to charts, medication cards, treatment plans, and care plans.
  • Chart nurses’ notes accurately and descriptively, including care provided and resident responses.
  • Record diet orders and report discrepancies involving physician orders, diet changes, or charting errors.
  • Prepare and administer physician-ordered medications and treatments after verifying resident identity.
  • Maintain adequate medication, supply, and equipment stocks; order prescribed medications and supplies as needed.
  • Maintain accurate narcotic records, report discrepancies, review medication cards, and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend personnel actions to the Nurse Supervisor.
  • Report staffing shortages and absentee call-ins to the Nurse Supervisor.
  • Develop work assignments and assist personnel with assigned work.
  • Provide leadership to nursing personnel and make daily rounds to monitor performance against nursing standards.
  • Ensure nursing assistants and nurse aide trainees meet training and supervision requirements.
  • Meet regularly with nursing personnel to identify and correct problem areas and improve services.
  • Receive and give nursing reports at the beginning and end of shifts.
  • Report occupational exposures, fraud, and other incidents according to facility procedures.
  • Inform personnel about new admissions and prepare rooms for incoming residents.
  • Orient new residents and family members to the facility and make physician rounds as necessary.
  • Arrange diagnostic and therapeutic services as ordered and consult physicians regarding resident care, treatment, and rehabilitation.
  • Review and maintain care plans, monitor treatments, and evaluate residents’ physical and emotional status.
  • Provide or ensure appropriate direct nursing care, including catheterization, tube feedings, suction, dressings, colostomy and drainage-bag care, specimen collection, vital signs, range-of-motion exercises, and post-mortem care.
  • Use restraints when necessary and according to policy.
  • Notify physicians and next of kin of accidents, incidents, or changes in residents’ conditions.
  • Coordinate social and activity programs with nursing schedules.
  • Arrange transportation and escort discharged residents, notify families of deaths, and contact funeral homes when requested.
  • Develop, plan, conduct, and schedule in-service training and orientation programs for nursing personnel.
  • Assist with clinical supervision and training of nurse aide trainees and attend required continuing education and facility training programs.
  • Monitor personnel compliance with safety, sanitation, infection-control, hand-hygiene, fire-safety, and disaster-preparedness procedures.
  • Maintain clean and safe nursing areas and ensure appropriate use of personal protective equipment and safety equipment.
  • Participate in infection-control programs and procedures for communicable diseases and isolation precautions.
  • Recommend and maintain equipment and supply levels and ensure equipment is operated safely by trained personnel.
  • Review resident care plans, coordinate MDS assessments and revisions, and ensure nursing notes reflect care-plan implementation.
  • Provide data to the Quality Assurance and Assessment Committee and report suspected fraud involving false billings, cost reports, or kickbacks.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. Most recently, Hendrickson served as CFO of U.S. Physical Therapy, overseeing 779 outpatient clinics across 44 states. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego. Dr Conway currently serves as CEO of Optum, UnitedHealth Group's health services division with revenues exceeding $200 billion.
