Requirements
  • A Nursing Degree from an accredited college or university, or graduation from an approved LPN/LVN program.
  • A current, unencumbered, active license to practice as an LPN/LVN in the state.
  • Current CPR certification.
  • Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to nursing care facilities.
  • Knowledge of and experience with PCC is preferred.
  • Required continuing education and licensing must be maintained.
  • Good standing with the State Board of Nursing must be maintained.
  • Ability to read, write, speak, and understand English.

Responsibilities
  • Provide direct nursing care to residents and supervise day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and guidelines.
  • Ensure nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Review department policies, procedure manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Prepare written and oral reports concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the nursing service quality assurance program and facility surveys.
  • Review and update resident discharge plans and assist with nursing-service discharge planning.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents and complete associated forms and reports.
  • Complete medical forms, reports, evaluations, studies, charting, census reports, and resident charge slips.
  • Protect user credentials, resident protected health information, and facility information systems from unauthorized access or disclosure.
  • Complete and file records and charts for resident admissions, transfers, and discharges.
  • Receive telephone orders from physicians and record them on physician order forms.
  • Transcribe physician orders to resident charts, Kardex, medication cards, and treatment or care plans.
  • Chart nursing notes and accident or incident reports accurately and descriptively.
  • Record diet orders and report discrepancies involving physician orders, diet changes, or charting errors.
  • Prepare and administer physician-ordered medications and treatments after verifying resident identity.
  • Maintain medication, narcotic, supply, and equipment stock levels and report discrepancies or needs.
  • Review medication cards for completeness and accuracy and manage automatic stop orders and medication disposal.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend personnel actions to the Nurse Supervisor.
  • Report staffing shortages and absentee call-ins.
  • Develop work assignments and provide leadership to assigned nursing personnel.
  • Make daily rounds to ensure nursing personnel perform assignments according to nursing standards.
  • Ensure nursing assistants are enrolled in or have graduated from approved training programs and that trainees are directly supervised by licensed nurses.
  • Meet regularly with shift personnel to identify and correct problem areas and improve services.
  • Maintain effective working relationships with facility departments and a calm, supportive unit environment.
  • Review personnel complaints and grievances and administer disciplinary action fairly.
  • Exchange nursing reports at the beginning and end of shifts and report occupational exposures or suspected fraud.
  • Inform personnel about new admissions and ensure rooms are ready.
  • Orient new residents and families, make physician rounds, and coordinate diagnostic and therapeutic services.
  • Consult with physicians regarding resident care, treatment, and rehabilitation.
  • Review resident charts and care plans and ensure prescribed treatments and direct care are properly administered.
  • Carry out restorative and rehabilitative programs.
  • Inspect treatment areas and maintain clean and safe nursing-service areas.
  • Provide professional nursing services including catheterization, tube feedings, suction, dressing changes, colostomy and drainage-bag care, blood collection, massage, range-of-motion exercises, and post-mortem care.
  • Use restraints when necessary and according to policy.
  • Collect sputum, urine, and other specimens for laboratory testing and record vital signs.
  • Monitor seriously ill residents and report changes in resident condition to physicians and next of kin.
  • Check foods brought by visitors against residents’ dietary allowances.
  • Meet with residents and families and arrange transportation and escorts for discharged residents.
  • Notify families of resident deaths, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule nursing-service in-service training.
  • Orient new employees to the shift, policies, procedures, position, and duties.
  • Standardize work methods and train personnel regarding blood and body-fluid exposure risks.
  • Assist with clinical supervision planning for nurse aide trainees.
  • Attend outside training, facility in-service, advance-directive, and continuing-education programs.
  • Monitor personnel compliance with safety regulations, dress codes, hand hygiene, infection-control procedures, fire-safety drills, disaster preparedness, and use of protective equipment.
  • Participate in infection-control programs and procedures for communicable diseases, isolation precautions, hazardous conditions, and equipment.
  • Ensure adequate supplies, equipment, personal protective equipment, and material safety data sheets are available and used safely.
  • Ensure only trained and authorized personnel operate nursing-service equipment and monitor efficient supply usage.
  • Review care plans daily, identify revisions, and ensure nursing notes reflect care-plan implementation.
  • Ensure certified nursing assistants understand and follow resident care plans.
  • Assist with planning, scheduling, and revising the Minimum Data Set, including implementation of Resident Assessment Protocols and Triggers.
  • Provide data to the Quality Assurance and Assessment Committee.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego.
