- Current state Registered Nurse and Nurse Practitioner licensure.

- Provide comprehensive specialized palliative care to senior living residents in long-term care, assisted living, and independent living settings.
- Work directly with patients and families on end-of-life matters and goals-of-care planning for chronically ill patients.
- Independently complete patient histories and physical data, perform complete physical examinations, and interpret and integrate data to determine diagnoses and treatment plans.
- Facilitate serious-illness conversations with patients and families to establish goals of care and document preferences.
- Guide residents and families in completing advance directives, Physician Orders for Life-Sustaining Treatment forms, and other legal documentation.
- Work with the primary care team and senior living facility staff to identify gaps in care and provide education on palliative-care principles and practices.
- Collaborate with the primary care team to manage acute and long-term medical needs, serving as the symptom-management specialist for advancing serious illness.
- Provide expert symptom management for pain, fatigue, shortness of breath, nausea, anxiety, and other distressing symptoms.
- Prescribe appropriate pharmacologic and non-pharmacologic treatments.
- Order and interpret diagnostic and therapeutic tests based on patient needs.
- Maintain and accurately review patient records, charts, and other pertinent information.
- Instruct and educate patients and families about medications and treatment instructions.
- Exercise autonomy in medical decision-making and provide a broad range of diagnostic and therapeutic services.
- At least two years of Nurse Practitioner experience.
- Palliative care or hospice experience.
- Advanced Certified Hospice and Palliative Nurse certification, or willingness to complete it within one year of employment.
- Strong leadership, interpersonal, and problem-solving skills.

Enrollment in the Institutional Special Needs Plan, or I-SNP, will begin Oct. 1, with benefits taking effect Jan. 1, 2027. According to a news release, Austin-based Curana Health is the nation's largest provider-led primary care company serving senior living and skilled nursing communities. The plan will launch with Western Home Communities and other Iowa operating partners. "Align Senior Care Iowa reflects our long-term commitment to Iowa communities and is designed to bring those stakeholders together around a more connected care experience, one focused on improving access, reducing fragmentation and supporting residents with care models designed specifically for their needs," Dr. Brian Whyms, Curana Health medical director for Iowa, said in a prepared statement.
