Requirements
  • Candidates must have employment eligibility verification.
  • Candidates must have or be able and willing to obtain CPR and First Aid training.
  • Candidates must successfully complete a tuberculosis test, background screening, and drug test as conditions of hire.
  • Candidates must have reliable transportation, a valid driver's license, proof of insurance and ownership for personal vehicles used during work duties, and the ability to qualify for PATH's insurance coverage.
  • Applicants must be authorized to work for any employer in the United States; visa sponsorship is not available.
  • Regular and customary work must be performed within California.
  • Candidates must not currently receive services from PATH or have received services within the past two years.
  • Candidates should be able to gather and interpret data, identify trends, patterns, and insights, and produce accurate, precise work.
  • Candidates should be able to work effectively with colleagues, participants, and stakeholders and contribute to team goals.
  • Candidates should be able to communicate clearly and effectively, verbally and in writing, to individuals and groups.

Responsibilities
  • Conduct intake and individualized needs assessments for participants and collaborate with them to develop Individualized Service Plans that address housing goals, maintaining or increasing income, and personal goals, including medical, mental health, substance use, financial resources, vocational, and social support needs.
  • Provide referrals, linkages, information, and support to resources that help participants achieve their Individualized Service Plan goals.
  • Complete program-specific assessments, such as VI-SPDAT.
  • Evaluate participants' strengths and challenges in addressing short- and long-term goals, and conduct a 90-day review with each participant after initial intake.
  • Assist clients through all phases of homelessness, including street, interim housing, transitional housing, and permanent housing; help them access and maintain healthcare, social services, interpersonal support systems, and navigate the coordinated entry system.
Desired Qualifications
  • Two years of experience in healthcare, case management, homeless services, working with vulnerable populations, or a related field.
