Requirements
  • An active Nurse Practitioner or Physician Assistant license or credential and at least four years of direct clinical practice are required.
  • Experience in oncology, home care, palliative care, or primary care is required.
  • Comfort caring for cancer patients in person and virtually using video and web-based call software is required.
  • Ability to contribute to clinical problem solving, prioritize initiatives, and complete urgent and important tasks is required.
  • Experience participating in building or launching new programs and comfort adapting processes to improve patient care are required.
  • Ability to lead the team while also providing direct patient care and switching contexts as needed is required.
  • Ability to work in person and deliver in-home care to patients in Rhode Island and potentially southeastern Massachusetts is required.
  • Availability to work full-time, four days per week, is required.
  • A valid driver's license and reliable personal vehicle are required.
Responsibilities
  • Provide direct patient care in homes and through virtual platforms.
  • Conduct patient assessments, place orders, prescribe medications, document appropriately, and assign tasks to the broader care team.
  • Provide navigation and direct clinical care for high-acuity patients while supporting the broader patient population.
  • Collaborate with oncology practices through daily standups with their clinical leadership teams.
  • Collaborate with the clinical operations team to refine and implement the care model.
  • Deliver appropriate care at the appropriate time to patients with a cancer diagnosis.
  • Address clinical needs including symptom management, in-home urgent care, palliative care, mental and behavioral health care, and non-clinical social and related needs.
  • Help establish an evidence-based oncology care and navigation model and improve processes as Daymark expands into new markets.
Desired Qualifications
  • Oncology Nursing Society certification.

Daymark Health partners with health plans to take on total cost of care risk and provides supplementary cancer care through in-home and virtual services. Its platform blends care navigation, 24/7 support, symptom management, and early palliative care via a twice-weekly symptom tool, delivered by a community team of nurse practitioners, nurses, social workers, and Health Partners. The model is tied to payer contracts and aims to keep patients outside traditional clinics, reducing fragmentation and coordinating care across the health system. The goal is to improve patient outcomes while lowering overall costs for payers and patients, addressing the needs of the roughly two million Americans diagnosed with cancer each year.
