About the job
- Must be currently licensed as a Registered Nurse in the State of Wisconsin.
- At least two years of experience in Medical-Surgical, Home Health, Hospice, or Long-Term Care nursing is preferred.
- Must be sensitive to the needs of older adults.

- Identify, provide for, and accept responsibility for fulfilling patients’ total nursing needs, including assessment, medication management, documentation, supervision of other nursing personnel, and serving as care manager.
- Maintain and actively promote effective communication with all individuals.
- Maintain a positive image of the organization in the community in alignment with its mission, vision, and values.
- Provide on-call services as assigned.
- Participate as part of the core team providing care and services, and coordinate implementation of the plan of care.
- Coordinate all aspects of patient care in accordance with current professional standards and practice.
- Provide education and counseling to patients and, as needed, their families, consistent with the plan of care.
- Participate in Home Health and Hospice-sponsored in-service training and continuing education opportunities.
- Complete documentation within policy timelines, including ongoing observation and assessment of patient and family needs.
- Lead, guide, and direct others in providing quality care.
- Practice infection control mitigation at all times.
- Maintain honest, open, and professional communication with others, including promoting the interdisciplinary team.
- Respond by phone to patient and family calls in a timely manner.
- Perform other duties as assigned.
- Provide on-call coverage of up to one shift per week, Monday through Thursday from 4:00 p.m. to 8:00 a.m., and up to every fourth weekend from Friday at 4:00 p.m. to Monday at 8:00 a.m.
- Travel within a one-hour radius.
- Certification in Gerontology, Medical Coding, Wound Care (WCC), or Hospice and Palliative Nursing is preferred.
